Job summary

Falkland Surgery are looking for a new enthusiastic Clinical pharmacist to join our practice team.

We are a friendly, forward-looking 9 Doctor training Practice with 14,000 patients, looking to recruit a 2nd pharmacist as an integral part of our clinical team. Our progressive and innovative approach to the role of the Practice Pharmacist offers an exciting opportunity for the right applicant to develop both their own skills, within a well-established Pharmacy team.

Main duties of the job

The ideal applicant will have excellent organisational and communication skills, relevant clinical skills and experience, and a clear enthusiasm for working as part of a cohesive primary care team.Prior experience in General Practice would be desirable. We would prefer an independent prescriber.

The job will involve

Medicine management of acute and repeat requests and advice for patients and allied staff.

Reconciliation of discharge medication and outpatient letter requests.

Medication reviews and structured medication reviews via telephone, in person and care home ward round.

Management of patient safety issues and monitoring/recalls.

Up to date clinical knowledge and understanding of primary care systems and prescribing we use EMIS computer system.

Independent prescriber status.

Ability to take on patient facing roles for medication reviews and queries and promotion of self-care.

Work with the local medicine management team to carry out audits and medication switches.

Having involvement in ongoing training and education of allied staff and GP trainees

The Falkland Surgery has an excellent multidisciplinary team consisting of pharmacy team, paramedic, MSK practitioners, MH nurse, midwife, frailty matrons as well as very experienced practice nursing team

Job responsibilities

JOB SUMMARY

The practice pharmacist will be responsible for support to the practices with medication related matters, including reviews, queries and the delivery of the repeat prescription service. They will be actively involved in supporting medication reviews of patients in primary care with Long Term Conditions, Multi-morbidity and Frailty, including mitigating the clinical risk of their medication.

They will also be part of the group that implements effective medicines optimisation within the practice, identifying areas for improvement, and initiating and managing change.

They will be an active participant in the network of pharmacists for mentorship, continued professional development, and to ensure that interfaces between community pharmacy, GP practices, acute and community trusts and the Medicines Optimisation Team are productive and effective.

JOB RESPONSIBILITIES

Patient services

To provide a medication review and query services to patients via the telephone or face to face clinics in the practice. Where suitable to provide domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.

Provide professional specialist medicines management advice to GPs, practice nurses, community pharmacists, community nurses and other health professionals to promote the safe, effective and high quality, cost effective use of medicines in all prescribing decisions.

To provide pharmaceutical advice to the Patient Services Team on acute, recurring or repeat prescriptions.

To oversee medicines management in Long Term Conditions during clinics, including the overall management of medication related issues in those clinics.

To support GPs and other clinicians in addressing polypharmacy and medication compliance issues across the range of patients to whom this applies (i.e. patients with Long Term Conditions, Multi-morbidity, Frailty, Dementia, Learning Disability)

To support the practice with medication related work at the interface between the practice and acute/community trusts (e.g. following hospital clinics or discharge) or community pharmacists (eg weekly blister pack dispensing, or unclaimed prescription action.) This includes ensuring safe systems for medication reconciliation at these interfaces.

To help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.

To present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines.

To produce patient information leaflets and posters and run medicine awareness projects throughout the year.

Assist Partners with the appropriate monitoring and management of their prescribing budgets and achieving the aims of the BOB Medicines Optimisation process.

Undertake risk assessment and management and ensure compliance with medicines legislation and best practice for monitoring and ongoing prescribing of medicines.

Lead on medicine optimisation in line with the National Institute for Health and Care ( NICE) recommendations

Increase quality and safety of prescribing through audit.

Manage practice formularies to improve the quality, safety and cost effectiveness of prescribing in line with local and national schemes and QIPP

Implement drug withdrawals and alerts i.e. MHRA aimed at improving patient safety.

Contribute to multidisciplinary reviews

Review appropriate pathology results for patients on known medication.

Improve on prescribing practice through education

Report any safety/prescribing concerns

To update and maintain accurate patient medication records on the practices clinical computer systems including advice given and action taken.
