Job summary

Due to retirement and relocation Washway Road Medical Centre is looking for an Advanced Nurse Practitioner to join the team.

Main duties of the job

The Nurse Practitioner will act as an autonomous practitioner within the scope of practice providing care for patients face to face, via telephone and online consultation and home visits.

The successful applicant must hold a V300 qualification minor illness qualification and will manage patients presenting with a range of acute medical conditions.

The postholder will use skills, knowledge and competencies as a qualified Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for pathology requests, treatments, referral, admission and discharging patients as appropriate.

An interest in the management of chronic disease, such as diabetes or respiratory conditions would also be beneficial.

About us

Our team consists of committed and flexible of 7 GP Partners (one retiring), 5 Salaried GPs, a pharmacist, multi-skilled nursing team including Nurse Practitioner's, Practice Nurses and Health Care Assistants.

We are an active member of a collaborative and supportive PCN and our additional primary care team includes First Contact Physiotherapists, Pharmacists, Mental Health Nurse and Social Prescribers.

Experienced and helpful administration team including reception, secretaries, workflow optimisation and medicine management.

We use EMIS web, ASMyGP, Docman and AccuRx technology

Job description

Job responsibilities

The advanced nurse practitioner will be responsible for providing first line care to patients from initial assessment, investigation and diagnosis to treatment and the evaluation of care, health promotion as well as supporting the management team in the reviewing of clinical policy and procedure. The successful applicant will work as an autonomous practitioner and must be a qualified prescriber.

The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Manage patients presenting with a range of acute and chronic medical conditions

b. Implement and evaluate individual specialised treatment plans for acute and chronic disease patients and refer onto secondary care where appropriate

c. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health

d. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. Develop, implement and embed health promotion and well-being programmes

f. Request pathology services as necessary

g. Process and interpret pathology and other test results as required

h. Maintain accurate clinical records in conjunction with extant legislation

i. Ensure read codes are used effectively

j. Maintain chronic disease registers and support the practice with annual review for chronic disease such as diabetes and respiratory conditions. A special interest in diabetic care would be advantegous

k. Prioritise health issues and intervene appropriately

l. Support the team in dealing with clinical emergencies

o. Recognise, assess and refer patients presenting with mental health needs

p. Extended and supplementary prescriber, adhering to extant guidance

q. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

r. Contribute to practice targets (QOF etc.), complying with local and regional guidance

s. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

t. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

u. Support the clinical team with all safeguarding matters, in accordance with local and national policies

v. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
