Job summary

The future is exciting. Are you ready to join Primary Care Sheffield (PCS) as one of our Advanced Nurse Practitioner growing collaboration within health and social care in Sheffield provides PCS with a wide range of opportunities which can improve patient pathways and reduce cost.

This post is for10.5 hours per week.

There is also scope to work at the Anima Hub, for up to full time hours per week (37.5).

The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the PSC practices to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required.

In order to work at this level NMC requirement for advanced practice must be met. To be considered for this exciting role you must have: a minor illness qualification and a Non Medical Prescribing qualification (V300)

The work to be undertaken, could be as part of a group wide triage team or within one of our seven practices.

Main duties of the job

  • Comprehensively assess, review and evaluate the needs of patients with undifferentiated healthcare needs

Primary Care Sheffield (PCS) is a GP-led organisation. Our shareholders are 75 GP practices in Sheffield covering over 600,000 patients. As well as providing a unified voice for general practice in the city, we play an active role both within our Accountable Care Partnership (ACP) and the wider South Yorkshire and Bassetlaw Integrated Care System (ICS).

PCS run nine practices in the City Darnall, North Darnall, Highgate, Mulberry, City, Sothall, Harrold Street, Buchanan Road, and Heeley Green which delivered care to 50,000 patients, approximately 7.5% of Sheffields total population. The practices are in some of the most deprived areas of the city with four of them located in wards which rank in the most deprived decile nationally.

Comprehensively assess, review and evaluate the needs of patients with undifferentiated healthcare needs to improve their physical, psychological and mental health wellbeing to promote self-care and reduce the need for hospitalisation. Including the needs of Children from the date of their first birthday.

Undertaking clinical assessment and providing the necessary treatment including the use of Independent Prescribing in line with local/national policies, procedures and guidelines. This may be face to face or via other remote communication methods (such as Telephone triage)

Request diagnostic tests only where clinically indicated in line with organisational, local and national guidelines and were appropriate.

Develop a plan of care in conjunction with the patient, with the application of clinical knowledge and, analysis of symptoms and data, identification of risk factors associated with those conditions, recognition of early signs of acute illness, involving the patients and the carers in the care plan and ensuring it is understood, documenting safety netting plans and processes

To utilise effective communication to aid onward referrals to specialists as appropriate. Facilitating a coordinated primary healthcare response to service users needs

To work in an autonomous manner without direct supervision whilst recognising limitation of the practitioner role

To carry out enhanced clinical skills as required.

To keep accurate and immediate records of all interventions fully utilising software systems in place (namely System one and C- The Signs).

To encourage clients to carry out self-care as appropriate through effective communication of health care using written information as appropriate

To maintain current professional registration and to ensure that all personal responsibilities in relation to this and to the revalidation process are met, including maintenance of a professional portfolio, evidence of practice hours, professional development planning and reflective writing.

To meet the organisational requirements for mandatory training appropriate to the role.

To recognise own professional limitations, practising accordingly, seeking advice and support as appropriate.

To take an active role in the organisational appraisal process and develop an appropriate and relevant personal development plan and work-based objectives.

To have awareness of the relevant organisational and professional Safeguarding policies and guidance and to act in accordance with their direction, attending update training as required.

The post holder will have regular 1:1s with line mangers to support ongoing development and review performance.
