Darnall Wellbeing are seeking to recruit a Social Prescribing
Link Worker who will have oversight of the day to day running of our SPLW team,
taking a lead on the delivery, management and development of the SPLW service.
Working within the Clover Practices you will
enable their patients to access a range of services and activities that
positively impact on their wellbeing and build connections within their
communities.

Main duties of the job
The Social Prescribing Link Worker will receive referrals from Clover GP
practices within the Seven Hills Primary Care Network, providing practice
(bookable clinics) and community-based consultations with patients.
The SPLW will deliver a range of activities that includes one-to-one
work and leading on group activities that improve the health and wellbeing of
people living in the area served.
They will work
closely with health professionals, service providers, local community,
voluntary and faith organisations to enable clients to achieve improved and
sustainable physical and mental health and wellbeing.
SPLW will
offer structured support for clients to change behaviour(s) and increase
capacity and capability to make changes. This will be achieved by building the
clients motivation, confidence for change, delivering self-care strategies,
connecting with community assets and through setting and supporting clients to
achieve goals including; increasing social connections, financial stability,
movement closer to/retention in work, improving relationships, support clients
to quit smoking, reduction in alcohol consumption, weight management,
increasing in physical activity etc.
SPLW
will work with other local service providers to maximise the potential health
and wellbeing outcomes for clients
The SPLW worker
will also have an agreed lead role within the team.

Job responsibilities
- Act
as the point of contact for patients accessing the service arranging appointments
by phone or in person as required
- Develop
a comprehensive knowledge of wider support services in the local and city wide area
and support individuals to access those that are relevant to their needs and work
on what matters to them
- Coordinate
the referral process from start to finish, communicating regularly with all stakeholders
- Record
monitoring data systematically on clinical systems, produce timely reports and
contribute to the overall evaluation
- Capture
key information and gather feedback on the impact of social prescribing on
health and wellbeing
- Attend
reflective practice and/or clinical supervision, team meetings and line
management meetings as required
- Build
strong working relationships with internal and external services in both the
statutory and voluntary sectors
- To
promote the link worker/social prescribing model
- Support
the other Social Prescribing Link Workers; providing advice, development
support, motivation and encouragement
- Provide professional peer support to the
SPLWs around complex cases
- Undertake annual staff appraisals and
bi-monthly supervisions
- Lead on project meetings to update
around work undertaken, feedback around unmet needs the team may be finding, what
may need development, what is working well and what are the challenges etc.
- Be the safeguarding lead for the Social
Prescribing Link Worker team
- Create and maintain referral routes into
the scheme via a wide range of frontline professionals in the health, social
care and community development sectors
- Give patients a voice, ensuring they
influence operational and strategic direction of the service.
- Be an active and contributing staff
member, attending meetings as required and working constructively as part of a
team to meet all organisational and project goals
- Attend mandatory training and abide by
all organisational policies and procedures, including Safeguarding & Child
Protection, Health and Safety and Equality and Diversity
- Attend appropriate continuing
professional development, training and networking events
