Job Title: Advanced Nurse Practitioner
Site: Group wide, location to be confirmed
Grade: Band F - £21.16 to 24.27 per hour
Job Summary:
To act as an autonomous practitioner working independently and in conjunction with other health care professionals to triage, assess, diagnose and treat the conditions of patients attending within primary care; initiating direct referrals as appropriate.
Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
The post-holder will provide education and training to other staff and students, as applicable.
They will also be expected to be involved in research, as and when the need arises.
Job Responsibilities:
The Advanced Nurse Practitioner works autonomously and is accountable for their own professional actions. The responsibilities of this role include:
Undertaking Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
Working as part of a multi-disciplinary team, assessing diagnoses and treating patients with a range of acute, non-acute and chronic medical conditions.
Assessing, diagnosing and treating patients as appropriate, over the telephone and by video consultation who require acute medical attention.
Referring patients to other medical specialities for assessment if deemed appropriate.
Referring patients, as appropriate to other members of the practice team, secondary care and to other statutory and voluntary organisations.
Prescribing within competency and local guidelines.
Undertaking and interpreting a range of diagnostic tests and routine clinical procedures such as cytology, venepuncture and blood pressure monitoring.
Making critical judgements of the highest order to satisfy the expectations and demands of the job e.g. first contact with patients presenting acutely and manages care in the presence and absence of protocols.
Working within professional guidelines and codes of conduct.
Working within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning and risk management.
Provides and receives highly complex, sensitive or contentious information and frequently deals with situations which are hostile or emotive.
Special Requirements of the Post:
- Self-resilience to deal with emotional subjects raised by patient group and to know when to seek support from the team and others for debrief.
- An understanding, acceptance and adherence to the need for strict confidentiality.
- Ability to use own judgment, resourcefulness and common sense.
- A liking for working as part of team with close interactions with others.
- A commitment to maintain a high professional standard of care and keep up to date with all aspects of nursing care relevant to the post.
- A commitment to ensure all Health and Safety requirements and Infection Control measures are met and to report any problems to the Support Manager.
- A commitment to the effective use of Practice and NHS resources.
- An awareness of own limitations and experience.
- To work only in accordance with the UKCC Code of Conduct and within the Scope of Professional Practice.
- Membership of appropriate professional bodies.
