Job summary

SAPA5 Primary Care Network (PCN) is looking for a band 6-7 Occupational Therapist, 37.5 hours per week. This opportunity is offered as a secondment or permanent contract. The role would fit within the GP working day, currently Monday to Friday 8.00am–6.30pm, start and finish times are flexible.

Over the past year our Primary Care Network (PCN) has been developing a personalised care team which includes a part time clinical specialist (band 7) Occupational Therapist. SAPA 5 is a socio-economically deprived area, many people have complex needs bridging mental and physical health and challenging social circumstances, as such we need to expand the OT capacity of this team. We offer professional development and career progression (so if appointed as band 6 you could work towards band 7 and band 7 staff would be given the opportunity to develop towards ACP band 8a).

Main duties of the job

We are looking for an Occupational Therapist who has experience of working autonomously with people with mental health and complex needs. Primary care is an emerging area of practice, you would work, under supervision, within a new and exciting team built around the needs of patients in our part of Sheffield. You would continue to develop working practices to suit both patient need and your own circumstances. We now work with the mental health transformation project and would hope to influence a new way of working across all agencies involved in the emotional well-being of all in our city.

The post is open to Occupational Therapists who can be released on at least a 12-month secondment basis from their employing organisation or are able to take up a permanent or fixed term contract. We would require the applicant to have experience in chronic mental health. Experience or interest in some of the following would also be useful - deprivation, trauma informed practice, multi-morbidity, chronic pain, learning disability and dementia. The successful applicant would require excellent communication and networking skills, along with wanting to develop professionally themselves and contribute to the development of the team.

About us

SAPA5 PCN covers Shirecliffe, Southey, Parson Cross and Shiregreen areas in the North of Sheffield. It consists of 6 GP practices: Barnsley Road Surgery, Buchanan Road Surgery, Elm Lane Surgery, Green Cross Surgery, Norwood Medical Centre and Shiregreen Medical Centre. SAPA5 is the most deprived PCN in the city and so has many challenges. The practices are forward thinking, friendly, cooperative and 5 of the 6 are GP training practices.

Our patients have a considerable burden of physiological and physical mental health problems with a very high prevalence of long-term conditions and trauma, often experienced at a young age. This can impact on people’s ability to work and enjoy life. We know already how much difference occupational therapy can make to people’s lives and are excited about expanding and developing this team further.

  • Accountable for the delivery of high-quality therapy care across a primary care network.
  • Develop and contribute to clinical leadership to develop staff from a variety of non-professional backgrounds within the remit.
  • Contribute to evaluation and development of clinical practice within the primary care ensuring compliance with national and local guidelines and frameworks.
  • Contribute to developing the evidence base for practice through audit, clinical effectiveness and research activity and evaluation of OT service.

Main Duties and Responsibilities

Clinical Practice as a Specialist / Highly Specialist (dependent on experience and banding) Autonomous Professional

  • Carry a clinical caseload including complex cases requiring advanced clinical reasoning and (highly) specialist therapy, planning and prioritising own work.
  • Delegate clinical duties to clinical and support staff and ensure they have appropriate knowledge, understanding and competence. Provide training and supervision for the task if required.
  • Gather, analyse and evaluate information to identify problems in partnership with the patient, carer and colleagues.
  • Select and use appropriate assessment techniques. Make clinical diagnosis and apply clinical reasoning.
  • Negotiate, agree expectations and set appropriate, realistic and individualised therapy goals in partnership with the patient, carer and colleagues.
  • Plan, implement, and evaluate occupational therapy intervention plans, which address why patients, groups and communities are not able to engage in occupations that have health benefits, and which aim to increase patients’ occupational performance and engagement
  • Empower patients, families, and groups to self-manage through shared decision making, health coaching or motivational interviewing, solving complex problems in innovative ways using occupation and outcomes-focused goals.
  • Enable and advise patients, and their families or carers, about how to engage in occupations that promote physical, psychological, and emotional health including organisations and/or equipment that can help with reduced functional ability for daily activities
