Job summary

This is a responsible, varied and challenging role, suitable for someone who works well under pressure, is self-motivated and can multitask, following our standard operations processes whilst still working to a very high standard, Working strictly in accordance with specific Hub protocols in the delivery of prescribed programmes of patient care.
Main duties of the job
Job responsibilities:
- Work as part of a multi-disciplinary team to deliver safe and high quality care.
- Mainly delivers the flu vaccine campaign in line with practice, national and local protocols
- Where/when experienced and trained deliver:-
- Phlebotomy/Venepuncture
- BP Testing/Monitoring
- Urinalysis
- Chaperoning duties
- Smoking Cessation Advice/monitoring
Care Management
· Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking.
· Monitor and meet care outcomes against standards and recommend or initiate changes as necessary.
Health Improvement
· Management of health screening programmes and communication to other team members to raise awareness of health priorities and screening objectives and programmes.
Administration and professional responsibilities
· Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures

As well as providing a unified voice for general practice in the city, we play an active role both within our Accountable Care Partnership (ACP) and the wider South Yorkshire and Bassetlaw Integrated Care System (ICS).
