Care Coordinator

28 hours per week Starting Salary £21,930 pro rata

The Clover Group as part of the Sevnhills + PCN has recently won a HSJ Primary Care Innovation Award for its Covid-19 Cold Visiting Service. Taking forward the principles of delivering proactive care for our most vulnerable patients, the Clover Outreach Visiting Team (COVT) has been created. This role will be a key role in this team.

Main duties of the job

A Care Co-ordinator (CC) may be required to deal with patients and, if appropriate, their carer, before and after the patients consultation with a clinician or other healthcare professional.

Working closely with the patient and their clinician or other healthcare professional, the CC co-ordinates patients healthcare and directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

The CCs role requires them to be able to work with, and understand the roles of, a variety of different people working in the practice and across the practice including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, and pharmacists.

About us

Primary Care Sheffield (PCS) is a GP led organisation with all independent Sheffield practices holding shares in the company. We aim to improve patient care through a unified supportive approach to general practice. We directly manage seven very different practices in the city, which sit in 4 Primary Care Networks.

We also provide workforce solutions to practices in the city that include clinical pharmacists, first contact physiotherapists and ECPs. We work with the universities and HEE to support employment opportunities for a range of post registration groups in general practice.

PCS holds a full seat at the city wide Accountable Care Partnership, alongside the three Foundation Trusts, the CCG and the City Council. We have excellent working relationships with each of these organisations and along with the CCG and the LMC we have ensured that general practice will become increasingly important in the development of health and care services in the city.
