    Primary Care Sheffield Ltd is owned and led by the city's GPs and was established in 2015 to improve patient care through a unified supportive approach to General Practice.

    This role is being part-funded by NHS England as part of the GP Five year framework.

    About us

    Primary Care Sheffield currently employs an established team of pharmacists within a robust and peer supported network and are looking to expand further due to interest by a significant number of GP practices and Networks within the city.

    We are looking for several clinical pharmacists who will work as part of a multi-disciplinary team in a patient-facing role in Sheffield general practices including:

    • Optimise the use of medicines

    • Improve the safety and quality of prescribing

    • Perform specific elements of general practices clinical workload

    • Deliver long term conditions and Medication Review Clinics of specific Clinical areas

    • Provide face to face medication reviews

    • Providing patient education on self- management

    • Management of repeat prescribing processes; prescription reauthorisations and management of medicines on transfer of care.

    • Opportunities to contribute to the further development of the services.

    Job responsibilities

    Patient facing long-term condition clinic

    See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma).

    Patient facing clinical medication review

    Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.

    Patient facing care home medication reviews

    Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.

    Work with care home staff to improve safety of medicines ordering and administration.

    Patient facing domiciliary clinical medication review

    Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.

    Attend and refer patients to multidisciplinary case conferences.

    Management of common/minor/self-limiting ailments

    Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence.

    Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.

    Patient facing medicines support

    Provide patient facing clinics for those with questions, queries and concerns about their medicines.

    Telephone medicines support

    Provide a telephone help line for patients with questions, query and concerns about their medicines.

    Medicine information to practice staff and patients

    Answer relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines.

    Suggest and recommend solutions.

    Provide follow up for patients to monitor the effect of any changes.

    Desirable

    • Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience

    • Minimum of 2 years post-qualification experience
    • An understanding of general practice and how it works
    • An understanding of the context of prescribing in primary care and strategies for its improvement
    • Excellent interpersonal, influencing and negotiating skills including within patient consultations
    • Excellent written and verbal communication skills
    • Ability to implement medicines optimisation
    • Good IT skills including the ability to obtain and analyse complex technical information
    • Recognise priorities when problem solving and identify deviations from the normal pattern, able to refer to seniors or GPs when appropriate
    • Able to work under pressure, prioritise workloads and to meet deadlines.
    • Work effectively both independently and as a team member
