Are you a self-motivated, enthusiastic and adaptable individual? Ireland Wood and Horsforth Medical Practice are seeking a full-time Practice Nurse. Previous GP practice experience is desirable.
We are a multidisciplinary practice that includes GP's, Nurses, Healthcare Assistants, Clinical Pharmacists and Physiotherapists.
Main duties of the job
The post-holder is
responsible for working as a member of the Nursing Team in all aspects of their
clinical and administrative activities, both in the surgeries and patients
homes. Together with the administrative team, they will ensure that
the interactions between patients and other visitors to the practice and practice team are to the standards agreed.
The post-holder will be required to work across both sites.
To be fully proficient in Chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care including compression, as well as other general treatment room duties.

Job responsibilities
- Assesses the nursing needs of patients and provides a quality nursing service based on research evidence and practice within NMC guidelines
- Provides appropriate care/treatment in conjunction with the GP, Nurse Practitioner, or independently, as appropriate, and according to practice policy, guidelines, and protocol
- Provides care to those patients with chronic diseases including diabetes, asthma, and COPD, in addition to undertaking screening activities such as cervical cytology
- Refers patients to other medical and nursing colleagues where appropriate
- Manages computerised patient notes in the process of assessment of need and delivery of care. Evaluates the outcome of that care and makes changes to the care plan for individual patients
- Assists patients in their usage of practice equipment and the collection of specimens
- Assists with a training and induction plan in liaison with Senior Nurse for the Healthcare Assistant (HCA) and on-going guidance and direction in the day-to-day duties of the HCA.
- Monitors, contributes, and implements evidence-based guidelines within clinical practice with the emphasis on a nursing and holistic approach
- Implement, monitor and evaluate all NICE guidance within the practice
- Nursing duties involved are shown in the appendix. This is not an exhaustive list and will be adapted to reflect the needs of patients together with national and local policies e.g. Quality & Outcomes Framework (QOF)
- Act as chaperone support if required by doctors/nurses
- Receive and transmit information on the telephone to the agreed practice standard, with particular reference to confidentiality.
- Adherence to the requirements of the Quality & Outcomes Framework (QOF)
- Undertakes other clinical activities identified by
the GPs and Senior Nurse in accordance with written protocols.
- Undertakes any training identified as appropriate
to the grade.
- Identifies personal training needs and discuss them
with the HR & Operations Manager.
- Contributes to the development of practice
protocols, monitors and evaluates the implementation of practice
protocols.
- Has the opportunity to be the lead person in
mutually identified clinical areas/topics.
- Conducts audit of nursing care where appropriate
and evaluates the outcomes of nursing activity
- Maintains a personal portfolio.
- Organise their own work schedule within the
constraints agreed by the GPs and Practice Manager.
- Keep up to date on nursing developments and changes.
- Be receptive and open to the implementation of new
ideas and the change process.
