Job summary

An exciting opportunity has arisen for a position based in Hendford Lodge Medical Centre. We are seeking an enthusiastic, pro-active and forward-thinking Nurse Practitioner or Paramedic (ECP) who is looking to work in primary care and join our skilled multi-disciplinary team. Primary care experience and a prescribing qualification is desirable but not essential.

Main duties of the job

You'll form part of our acute care team who focus on predominately seeing patients for on the day and acute minor illnesses. Ideally, with the ability to prescribe autonomously and see patients during a 15 minute consultation time. You will need to have a clean driving license, as you'll be responsible for home visits each day. Its important that you can also see children for a range of illnesses.

A working day runs from 9:00am to 6:00pm, during this working day you would receive an 1 hour unpaid lunch break and two paid coffee breaks.

If successful you will receive the following:

6 weeks of annual leave (Pro rata for part time staff)

1 week per year study leave

Although we are looking for cover across a 5 day week, we can be flexible with working patterns and part-time roles will be considered.

Situated in central Yeovil, with a branch site on the outskirts at Abbey Manor Park; we are an established and dynamic Practice, with a list size of 14,300 patients. We are rated as Good by CQC and look to provide the highest level of patient care.We are also a training practice and take pride in developing our teams within a supportive environment

You will be joining a welcoming and highly experienced Clinical team of GPs, Acute Care clinicians, a Practice Pharmacist, Nurses, HCAs and Health Coaches; that in conjunction with our participation in the Yeovil Primary Care Network.

Job responsibilities

The post holder is an experienced Nurse Practitioner or Specialist Paramedic who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment including prescribing and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process but with the full support of the wider clinical team in making treatment plans. They will work collaboratively within the general practice team to meet the needs of patients and supporting the delivery of policy and procedures.

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Work with patients in order to support compliance with and adherence to prescribed treatments

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Support and manage health needs of women presenting for family planning, sexual health consultations

Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health

Conduct visits to patients in either their own home or a care home environment

Liaise with GPs to admit patients to secondary care if they are in acute medical need

Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other relevant care providers
