Job summary

Diamond Health Group are seeking a motivated Health and Wellbeing Coach to join our skilled multi-disciplinary team, to deliver a high standard of patient care. The role will involve identifying the needs of patients; supporting them to take pro-active steps to set goals and achieve their personalised health and care plan objectives.

This positon is up to 37 hours per week.

Main duties of the job

Working alongside our Health and Wellbeing Coaching team, you will:

  • Provide one-to-one and group heath coaching support by phone, video consult and face to face
  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals
  • Support local health, social care and voluntary sector professionals to make appropriate referrals to the service
  • Promote and raise awareness of the health coaching service
  • Work with our Primary Care Network colleagues and attend relevant MDT meetings, giving information and feedback on health coaching

About us

This position is accountable to the Yeovil Primary Care network, but you will be based at Diamond Health Group.

Diamond Health Group is situated in central Yeovil, with a branch site on the outskirts at Abbey Manor Park; we are an established and dynamic Practice, with a list size of approximately 15,000 patients. We are rated as Good by CQC and look to provide the highest level of patient care. We are also a training Practice and pride ourselves on the development of our teams, in a supportive environment.
