Job summary

Working as part of our medicines management team at the Practice, the postholder will play a key role in supporting the delivery of high-quality, safe, and effective medicines management for our patients. You will work with the support of a Clinical Pharmacist and alongside GPs, ANPs and the Repeat Prescription Administration team to improve prescribing processes, enhance patient care, and ensure the effective use of medicines within the practice. You will also support the practice in providing medication reviews and achieving Quality and Outcomes Framework targets.

Main duties of the job

  • Medicines Optimisation: Support safe, effective, and cost-efficient prescribing by reviewing and managing repeat prescriptions, reducing waste, and ensuring compliant practice protocols and best practices.
  • Clinical Support: Assist in medication reviews, reconcile medicines following discharge, and advise clinicians on the Pharmacy First consultation service and guidance on medications
  • Patient Engagement: Handle patient daily prescriptions queries (face to face, online or by telephone) and offer guidance on medication use, support long-term condition management, and promote patient self-care. Review and update medication changes from clinic letters.
  • Prescribing Support:ensure adherence to formulary guidelines, manage high-risk medication monitoring, and carry out audits to improve prescribing safety, quality and cost effectiveness.
  • Administration & Documentation: Maintain accurate records, update patient medication lists, and support compliance with CQC and NHS regulations. Review ways of working to maximise income from PPA and QOF, andmanage appropriate practice stock levels for emergency use and practice procedures.
  • Continuous improvement: Keeping up to date with professional responsibilities and CPD requirements and consider how they might contribute to the ongoing continuous improvement
  • Support for the medicines management team: Contribute to the effectiveness of our medicines management team by providing support and training as appropriate

About us

Park Medical Practice has a 11,300 list size. This role will join a team of a Clinical Pharmacist and three part time prescription admin staff, all supported by a GP Partner. Park is part of a proactive PCN with five practices who work collaboratively. We recent began the national PCN Test Pilot programme aimed at finding new ways of working to help meet demand for our services and improve patient care.

Essential

  • Registered with the General Pharmaceutical Council (GPhC) as a Pharmacy Technician
  • Relevant pharmacy qualification (e.g., BTEC/NVQ Level 3 in Pharmaceutical Science or equivalent)
