Job summary

We are seeking a highly motivated part time Care Co-ordinator/Administrator to join our thriving practice. We are looking for a friendly, organised, and enthusiastic individual to join our hardworking and committed healthcare team.

Job Purpose:

Working alongside the clinical and admin teams, the Care Co-ordinator will help patients in finding the right support with the right person, at the right time. Care Co-ordinators champion person-centred care and provide options and education to support patients in best utilising primary care services.

Care Co-ordinators support the co-ordination and efficient cascade of information to patients.

Please note the role of a Care Co-ordinator is not a clinical role.

Start date June 2025

Working 20 hours per week

4 hours per day (ideally 10.00am to 2.00pm) - Monday to Friday

This is a temporary contract to cover maternity leave

Salary depending on skills and experience.

Main duties of the job

Care Co-ordinators will be expected to:

Have a thorough knowledge of all practice procedures and to work in accordance of written policies

Undertake a variety of administrative duties to assist in the smooth running of the practice including the provision of administrative support to clinical staff and other members of the practice team

Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone

Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way

Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies

Complete other administrative tasks as directed by the Leadership
