Job summary

We are looking to recruit a new nurse to join our friendly and supportive team.

We are a new multi-disciplinary team of GPs, Paramedics, ANPs, Clinical Pharmacists, Practice Nurse, Nurse Associate, Health Care Assistants and Physiotherapists, working collectively and innovatively to deliver primary care to a diverse practice population. Our objective is to provide care holistically with a focus on high quality. Equally important to this, is our commitment to supporting one another.

You will be forward thinking, ideally with general practice experience and have a willingness to help shape our services for the future. We work closely with our PCN colleagues and have excellent relationships with our ICB.

Main duties of the job

Duties to include general treatment room tasks, immunisation, cervical cytology, wound care, long term conditions monitoring, health promotion, travel health, etc.

Salary Range:£28,500 to £42,500

Osmaston Surgery is a large practice of circa 14,000 patients, situated within Derby City Centre and is a member of Derby City North PCN. Job responsibilities

To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population. The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure. The Osmaston Surgery must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

  • NMC Registered Nurse Level 1

Desirable

  • Previous primary care experience
  • Mentor or teaching qualification
  • Post graduate diploma or degree (Chronic Disease Management)
  • Nurse prescriber
  • Family planning qualification

Experience

Essential

  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Experience of working autonomously
  • Experience of infection prevention and control measures

Desirable

  • Experience of working as a practice nurse or community nurse
  • Experience of managing elderly and housebound patients
  • Experience of quality initiatives i.e. benchmarking

Clinical Knowledge and Skills

Essential

  • Wound care / removal of sutures and staples
  • ECGs
  • Venepuncture
  • New patients medicals
  • Requesting pathology tests, processing results and advising patients accordingly
  • Understanding the importance of evidence based practice
  • Understand the requirement for PGDs and associated policy
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and understanding when to refer to GPs
  • Chaperone procedure
  • Women's health (cervical cytology, contraception etc)
  • Immunisations (routine, childhood and travel)
  • Chronic disease management
  • Spirometry

  • Travel medicine
  • Broad knowledge of clinical governance
  • Knowledge of public health issues in the local area
