Job summary

We are looking to recruit 2 motivated and enthusiastic prescribing clinical pharmacist to join our dynamic Primary Care Network (PCN) and work as part of a multi-disciplinary team within the East Kennet area of rural Wiltshire.

Our PCN is a collaborative group of GPs whose surgeries are focal points within their communities.

Main duties of the job

The Clinical Pharmacist will improve the quality and safety of care offered to our patients across the Primary Care Network.

The Pharmacist will have regular face to face contact with patients, assisting with reducing the workload for GP Prescribers and supporting patients with long term conditions. Reconciling hospital discharge summaries, ensuring continuity of medication and updating medication records post discharge will be key aspects to this role.

Joining two existing Clinical Pharmacist's within the PCN, you will have the opportunity to work within a varied multi-disciplinary team across 4 practices to enhance the primary care offering to all of our patients.

Job responsibilities

The Prescribing Clinical Pharmacist will improve the quality and safety of care offered to our patients across the Primary Care Network. The Pharmacist will have regular face to face contact with patients, assisting with reducing the workload for GP Prescribers and supporting patients with long term conditions. Reconciling hospital discharge summaries, ensuring continuity of medication and updating medication records post discharge will be key aspects to this role.

Main Duties:

Repeat Prescribing

Support all Practices with reviewing, proposing and actioning repeat prescribing medication policies locally.

Long Term Conditions

Undertake face to face or telephone consultations with patients to review long term conditions where medicine optimisation is required. These reviews will be undertaken alongside the Practice Nursing Teams. Review the ongoing need for each medication, monitoring requirements and support the patients with utilising their medication correctly.

Structured Medication Reviews

Undertake medication reviews with patients, review and request monitoring tests as appropriate for patients both in the Practice. Undertake clinical medication reviews with patients in care homes and produce recommendations for the relevant Practices on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration.

Telephone Medicines Support

Provide a telephone help line for patients with questions, queries and concerns about their medicines.

Discharge from Hospital

To reconcile medicines following discharge from hospital into the community or care homes by identifying changes and updating the medical records appropriately.

Audits

Undertake periodic prescribing related audits as directed by the PCN Practices

Support

Provide support to the Practices with Prescribing Safety, QOF and guidance for Dispensary Team Members. Undertake periodic training sessions on topics of interest for Dispensary Team Members across the Primary Care Network.

Risk Stratification

Identify patients at high risk of harm from medicines through clinical system searches at Practice level.

Care Quality Commission

Work with each Practice to ensure the Practice is compliant with CQC standards where medicines are involved.

Safety

Implement changes to medicines that result from MHRA alerts, product recall and other local and National guidance.
