Job responsibilities
Job Summary
The postholder is an experienced nurse who is
responsible for ensuring the delivery of safe and effective patient care to the
practice population using autonomous clinical skills and a broad, in-depth
theoretical knowledge base. The postholder will manage a diverse clinical
caseload, addressing the needs of patients presenting within a primary care
setting.
The practice nurse will contribute to the range of nursing services offered by
the practice. Working independently, the postholder will take part in audit,
research, and the development, implementation and evaluation of local
standards, policies, guidelines, and care pathways.
The postholder must hold a current NMC registration and work within the NMC
Code of Conduct, adhering to agreed practice standards and protocols.
Main Responsibilities
- Patient
Care
Meet
patients nursing care needs as identified on assessment or where care has been
transferred via another clinician within the practice.
Deliver
high-quality, evidence-based care in line with practice policies and protocols.
2.
Health
Screening and Promotion
Initiate
and maintain health screening and health promotion clinics for identified areas
of the practice population.
Recognise
when modifications of systems or referrals are required to meet patient needs.
3.
Chronic
Disease Management
Manage
the nursing care of patients with chronic diseases, ensuring best practice and
evidence-based care.
Utilise
the practice computer system for inputting data, managing recalls, and
supporting screening programmes.
4.
Specialist
Clinics
Support
and/or lead on the following clinics:
- Asthma and COPD
- Diabetes
- Hypertension
- Cardiovascular disease
- Cervical cytology
- Travel vaccinations
- Learning disabilities
- Severe Mental Illness (SMI)
5.
Teamwork
and Liaison
Maintain
effective liaison across all areas of the primary care team and external
agencies to ensure continuity and coordination of care.
Participate
in team meetings.
6.
General
Treatment and Clinical Duties
Deliver
treatment room services including wound management, blood tests, ECGs, and
other general procedures.
Ensure
safe and effective clinical practice at all times.
7.
Health
Promotion and Patient Education
Provide
health promotion advice including nutrition, weight management, CHD and
diabetes care, smoking cessation, and exercise.
Assess
patients motivation and readiness for lifestyle change and refer to other
services as appropriate.
8.
Infection
Control
Support
infection control practices within the surgery and ensure adherence to local
and national guidelines.
Carry
out infection control audits, maintain hygiene standards, ensure proper
segregation and disposal of waste, and report any concerns.
Liaise
with relevant authorities and ensure regular audit and feedback cycles.
9.
Emergency
Management
Be
competent in anaphylaxis and resuscitation techniques.
Attend
annual updates and training as required.
10. General Duties
Maintain
accurate and up-to-date patient records using the clinical computer system.
Collect
data and participate in regular audits and quality improvement activities.
Ensure
timely logging and dispatch of specimens to secondary care.
Support
the practices health promotion and chronic disease management programmes.
11. Education and Professional Development
Maintain
continuous professional development in line with NMC revalidation requirements.
Participate
in training and educational updates as required by the practice.
12. Information Technology
Use
the computerised medical records and messaging systems to ensure effective
communication within the practice.
Extrapolate
and share relevant information from external correspondence and agencies.
Undertake
IT training as required to maintain competency.
13. Patient Liaison
Be
available to address patient queries that cannot be resolved by administrative
staff, while recognising own limits of competence.
Ensure
that all formal complaints are passed promptly to the Practice Manager for
appropriate action.

Essential
Qualifications and Experience Registered Nurse with current NMC registration.
Demonstrated competence in chronic
disease management.
Up-to-date resuscitation and
anaphylaxis training.
Understanding of infection control
and health promotion principles.
Excellent Computer skills
Understanding Health and Safety issues
Willingness to learn and acquire new skills
Desirable
Qualifications and Experience
Experience in a primary care
or community nursing environment.
Certificates in cervical
cytology, Vaccinations and immunisation, travel health and chronic disease
management (e.g. Diabetes, Asthma/COPD).
Experience using clinical IT
systems (e.g., EMIS, SystmOne).
Demonstrate ability to be
Involved in audit or quality improvement projects.
Undertaken mentorship training
or be willing to do so.
