Job responsibilities

To provide nursing services to a defined practice population with the assistance of, and in collaboration with the Practice Nursing Team and GP colleagues. This will include clinical duties, health promotion, health screening. To contribute in the development of services provided by the Practice and to participate in clinical audit. To work with the GPs and the Specialist Practice Nurses to develop and implement the Practice Development Plan, the National Service Framework targets and to meet and maintain the quality indicators contained within the new General Medical Services (nGMS) Contracts Quality and Outcomes Framework.

KEY RESPONSIBILITIES

Clinical

1. To work with the Specialist Practice Nurse to assess, plan and provide health care and nursing services to the practice population ensuring they are effective, relevant, evidence-based and of a high standard.

2. To work with the Specialist Practice Nurse to provide a comprehensive general practice nursing service, to include venepuncture (taking blood), wound care, diagnostic blood and urine tests, ear care and advice, new patient registration health checks, over 75year health assessments and travel immunisations and advice, within the surgery, and where appropriate in patient homes.

3. To work with the Specialist Practice Nurse and GP colleagues to achieve and maintain breast and cervical screening targets. To record and monitor results, including efficient and accurate call and recall systems.

4. To work with the Specialist Practice Nurse to provide an effective health promotion service, to include smoking cessation and healthy lifestyle clinics.

5. To work with the Immunisation Lead, to include the administration of the childrens immunisation programme according to current schedules, the accurate recording of the information in the child health records

6. To work with the practice team in order to administer the annual influenza and pneumococcal vaccination programme. To include the identification, targeting and immunisation of vulnerable patients within the practice population, to ensure that local and national targets are met.

7. To ensure that professional responsibilities are met as a practitioner supplying and administering medicines in accordance with a Patient Group Direction.

8. To assess patient understanding of treatment proposals, gain valid informed consent and works within the NMC Code of Professional Conduct to provide care for patients who lack the capacity to consent to treatment.

9. To work with the practice team to ensure clinical governance standards are met, including managing clinical risk within own clinics, active participation in clinical risk management meetings, and dealing with complaints.

10. To work closely with the practice team, primary care and community nurses, allied health professionals and social services colleagues by active participation in clinical meetings, to ensure effective lines of communication in order to achieve a high standard and continuity of care for the practice population.

11. To assist in emergency cleaning of bodily fluids when requested in line with the Practice Infection Control policy

Qualifications

Essential

• Registered General Nurse
• Recognised qualification to diploma level or above
• A demonstrable commitment to professional development
