NORTH BRINK PRACTICE

Primary care is changing and so is the way we deliver care to our patients. Practice Nursing at North Brink is evolving and the role now includes Nurse led Clinics which offers variety to the role and holistic care to our patients.

We are looking for registered nurses to join our well established team. You do not need to be experienced in practice nursing or primary care as we offer a full training program. We are also happy to consider newly qualified nurses.

North Brink Practice offers continued education and development to meet both your own personal development and the challenging needs of the Practice. You will receive a competitive salary dependent on qualifications and experience.

The role is full time position and will include evening and weekend working on a varied rotation.

Main duties of the job

In response to rising demand from patients and falling availability of GPs, North Brink Practice has recognised the potential for placing nurse-led care at the heart of our service. At North Brink Practice we recognise that nurse skills are far greater than patients often realise. So, we ensure that our patients regard our nurses as qualified healthcare professionals in their own right, and shatter preconceptions that the nurse is here simply to assist the doctor.

With a strong focus on developing skills in history taking and physical examination we enable our nursing team to work in acute clinics for both first-time problems and follow-ups so that North Brink nurses assess the healthcare needs of patients in the first instance and develop or deliver a care/treatment management plan to suit.

Of course, we do not expect you to be thrown in the deep end. All our clinics are supported and supervised by a GP that is always available and designated mentorship by senior/experienced nurses and Advanced Clinical Practitioners who have developed within the practice themselves.

North Brink provides continuous learning opportunities where nurse and GP colleagues can discuss their experiences, share learning points and identify any specific training needs either at the time or through clinical supervision and regular clinical update meetings. Immediate feedback focuses just as much on those patients who saw the GP as on those where the nurse managed the consultation independently.

We offer the opportunity to developfrom Practice Nurse through to Advanced Clinical Practitioner, and many of our nurses have developed areas of expertise. Whilst our nurse-led clinics form the backbone of the role, traditional skills such as immunisations, wound care and leg ulcer management, ECGs, drug administration, cervical screening we expect our new colleagues to develop the fundamentals of practice nursing through further learning, education and supervised practice.

Job Purpose

To perform as an autonomous nurse working both independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating direct referrals as appropriate supported by a GP comprising face-to-face consultations and telephone/electronic triage work

Provide general nursing care to practice population who are referred or self-refer in the practice or in home setting including assessment, planning, delivery and evaluation including:

o Wound care (assessment and management)

o Adult and Childhood immunisations

o Flu and COVID-19 vaccinations including home visits to housebound

o ECGs

o Pathology samples appropriate testing in surgery and sending to pathology when deemed necessary

o Cervical screening

o Health education and promotion

To trial and adopt new working methods designed to manage the increasing demand in primary care. Such methods are likely to require an increasing reliance on triage and consultation over the telephone and emerging technology

To provide education and training to other staff and students as appropriate and to take a lead role in encouraging teamwork and sharing best practice

To help develop the North Brink Practice for the overall benefit of its patients

MAIN DUTIES AND RESPONSIBILITIES

Clinical Responsibilities

Undertake clinical nursing practice using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services

Assess (with GP support after training and competent) patients presenting with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow up

Arrange hospital admission for appropriate cases

Undertake telephone triage work and telephone advice calls

Maintain accurate record keeping in SystmOne

Recognise the growing demand for primary care services and take positive steps to manage it

Create opportunities to address patients care needs in the most expedient way, taking into account that continuity of care and access to services are often in direct conflict

Influence patients to recognise the difference between their needs and their wants, and manage their expectations of what we can realistically deliver in a manner that is fair and proportionate for all care priorities

Make improved use of the telephone and other technologies. Encourage patients to rely on face-to-face consultation only where there is a clinical need to do so

Convince patients of your clinical competence and of the need to free up GP time for all but the most complex cases
