Job summary
Aintree Park Group Practice is a well established family Practice with two surgeries, one at Moss Lane, Orrell Park and the other at Oriel Drive, Old Roan. Our team of health care professionals'work with patients to achieve excellence in local healthcare'. We work closely with the District nursing team and other community services to provide the best healthcare for our patients. We also have wider support through the Primary Care Network. We are looking to recruit a Practice Nurse with a minimum of 5 years post registration experience, and at least 2 years primary or community nursing experience with an understanding of role
of the practice nurse.
Main duties of the job
- -Provide clinical
leadership in the delivery of practice nursing services to patients, ensuring
that the needs of the patient are the priority

Support the
nursing team in the planning and implementation of local guidelines, protocols
and standards, and of local projects or initiatives

Promote the role
of the nursing team in the provision of care

Participate in
practice team meetings, delivering the nursing agenda and participate in nurse
team meetings
- Take
responsibility for delivering long term condition management, GMS contract,
Direct Enhanced Services, Locally Enhanced Services, Quality and Outcomes
Framework, Liverpool GP Specification and Primary Care Network DES outcomes.

development and training of others in order to maximise staff potential

responsibility for own development, learning and performance including
participating in clinical supervision and acting as a positive role model,
delivering a quality service

Qualifications
Essential
- Educated and qualified to degree level
- Adult Nursing qualification
- Clinical supervision training and experience
- Independent and/or supplementary prescribing qualification
- Enhanced CRB check, the outcome of which must be satisfactory to the Practice.
- Professional portfolio
- Well motivated
- Work well as part of a team
- Ability to work under pressure
- Willingness to learn new skills and open to new suggestions
- Min 5 years post registration experience
- At least 2 years primary or community nursing
- Good time management skills and ability to prioritise work load
- Excellent communication and interpersonal skills, and an ability to communicate difficult messages to patients and family
- Evidence of IT and keyboard skills
- Hold current full driving licence and have access to a car
- Motivated, reliable, flexible and fit in with culture of the Practice Team

- Hold or working towards qualifications in COD, asthma, diabetes, COPD, cytology, family planning.
- Team leader experience
- Teaching and mentorship experience in a clinical setting
- Knowledge of public health issues.
- Ability to identify determinants on health in the area
