Job summary

An exciting opportunity has arisen for a highly motivated Assistant Manager to join Avon Valley Primary Care Network (PCN). We are looking for someone who is dynamic, hardworking and organised with great interpersonal skills to work collaboratively with both PCN team members and staff across our four member Practices.

You will work closely with the Clinical Director and PCN Manager to support the smooth running of our rapidly developing PCN to deliver patient-centred care to over 40,000 patients.

Travel will be a requirement of the post from time to time and as such candidates must have access to their own transport with full driving license.

The hours available are 37.5 hours per week. Salary is negotiable dependant on experience; approximately, £13.00 per hour.

Main duties of the job

As a highly motivated individual, the post holder will be expected to provide high quality co-ordination support across a range of services and projects delivered by Avon Valley PCN. This is a fast-paced environment that requires excellent organisational skills, supporting the coordination of multiple work streams.

The post holder must have good interpersonal skills and be able to demonstrate experience and confidence in areas of people and team management and show commitment to continued professional development for themselves and all PCN Team Members. Assist the PCN Manger in the management and development of non-clinical PCN Staff.

You will support the PCN Manager to ensure daily smooth running of the PCN, and deputise for the PCN Manager in their absence.

Job description

Job responsibilities

The Deputy PCN manager has operational responsibility for various functions of the PCN including:

· Responsibility for supporting the PCN Manager in the delivery and monitoring of projects and services committed to or commissioned by the PCN.

· Providing support to the PCN Manager financial management, resourcing, and regulatory compliance of the work of the PCN.

· Supporting PCN staff employed and hosted by the four member practice on behalf of the PCN.

· Arranging the strategic meetings across the year and weekly PCN Leads Meetings.

Finance

· Good financial and business acumen with sound understanding of NHS and other funding to maximise PCN (and practice) funding where possible

· Support the PCN Manager with oversight of the PCN monies, maximising potential and maintaining clear, auditable records to ensure tight control on budgets

· Accurate financial forecasting & timely reporting

PCN Workforce

· Conduct needs analyses for PCN employed staff in line with funding and contractual requirements, review as directed

· Work with the PCN Manager and Clinical Director to appoint new staff to the Additional Roles in line with the NHS England DES Specification

· Horizon scanning for future roles / funding / training

· Working with the PCN Manager and four member Practice Managers to ensure maximum funding is obtained from variety of budgets to meet the needs of the PCN

· Define job role, produce job description, person specification and advert

· Ensure all employees undertake required training, have regular appraisals

· Provide ongoing support to all employees

PCN DES Contractual requirements

· Support the Clinical Director and PCN Manager to ensure all services that the PCN is mandated to deliver are undertaken, regularly reviewed and delivered within the specified timeframe

· Staff are supported to deliver service specifications

· An annual report on the PCN delivery with expectations for the following year is developed and produced annually.

Clinical Director Support

· Attend wider strategic meetings across Hampshire as required.

· Create PCN Leads agendas, ensure actions fulfilled

· Support the Executive Manager in setting strategic goals for the PCN, developing action plans

· Scrutinise all PCN contracts (including IAGP) and work with the Data/Finance managers to ensure the PCN is delivering within required timeframe

· Provide briefings/synopsis for PCN members and Practice Partnership/Executive Group as required

· Support the SAH Finance Manager to provide financial analysis for PCN.

· Provide patient safety and wellbeing analysis for PCN

· Undertake regular reviews of resources to ensure these are matched to contractual requirements

· Review and provide briefing on alternative options or other contractual avenues for delivery

· Review options for aligning services and submit proposals eg visiting service, diabetes, respiratory

· Working with PCN to develop plans for delivery to match/exceed expected outcomes

· Responsible for managing external contracts with NHS and non-NHS third parties (eg social prescribing), ensuring KPIs and outcomes match to contractual requirement

· Review all data sharing agreements to support compliance with GDPR etc, provide advice and guidance to the PCN

Project Management

· Ability for strong management of projects on behalf of the PCN

· Ensuring projects brought in on time and on (under) budget

· Ensuring regular updates

· Support of external projects which will benefit the PCN

· Production of all project reports and end analysis reports
