Patient Care Navigator to join the team on a permanent basis.

Job responsibilities
MAIN DUTIES AND RESPONSIBILITIES
- Receiving patients and visitors, consulting with members of
practice team.
- Processing telephone requests for appointments, telephone
consultations and ensuring callers are directed to the appropriate healthcare
professional.
- Actively signpost patients to the appropriate point of care.
- Register new patients, temporary residents, and any requiring
immediate and necessary treatment, carefully checking all details for
accuracy and enter on computer registration link.
- Process patient deductions.
- Taking messages and passing on information.
- Assist
the office manager with target driven initiatives.
- Initiating contact with and responding to requests from
patients, other team member and associated healthcare agencies and providers.
- Filing and retrieving paperwork.
- Action personal and group tasks in a timely manner.
- Keep abreast of practice disseminated information via internal
and external communication methods.
- Attending practice meetings.
- Attend training sessions when required.
- Computer data entry/data allocation and collation; processing
and recording information in accordance with practice procedures.
- Providing clerical assistance to practice staff as required,
including word/data processing, filing, photocopying, and scanning.
- Providing clerical assistance to practice staff including
actioning practice workflow; generic email inbox and online documents
- Keeping the reception area, notice boards, leaflet etc. tidy and
presentable
- Cover sickness/annual leave and work reasonable overtime when
required.
- Understand the meaning of teamwork and work in collaboration
with your colleagues.
- Perform any other relevant and reasonable duties that may be
requested by the office manager or exec team.
- Undertake statutory and mandatory training as required.
- Book/edit/cancel appointments and recalls ensuring sufficient
information is recorded to retrieve medical record.
- Updating of the practice DNA record spreadsheet.
- Monitor effectiveness of the system and report any problems or
variations required.
- Deal with home visit requests, carefully noting all details
alerting the on-call GP to late visits.
- Retrieve and refile records as
required, ensuring that strict alphabetical order is adhered to.
- Ensure correspondence, reports,
results, etc., are filed in correct record.
- Raise and process patient referrals.
- Processing repeat prescriptions in accordance with practice
guidelines.
- Actively promote NHS online services and NHS App.
