Job summary

Heeley Plus Primary Care Network are a group of 8 practices who serve a population of around 48,000 patients in Sheffield. We are an active network and closely work with our local voluntary organisations and other partners to develop new models of care to benefit our patients.

We are seeking to develop a dynamic and progressive PCN Medical Support team. We recognise the importance of internal support and career development. As such we are seeking a Clinical Lead to work with a team nurse of junior PAs, nurse associates etc to support the delivery of network services as well as lead/support the development of the team. We seek applicants for the lead role with at least 3 years experience, and enthusiasm for team development, internal training, supervision and mentorship.

Our PCN is maturing well, we have an established leadership team of Clinical director and Network Manager, with good practice engagement and strong local partnerships. We are focused on integrated health and social care, with a focus on population needs, proactive prevention work. We support coaching approaches to building patient activation and upskilling internal teams, and our leadership team have national experience of leadership in this area.

Main duties of the job

The post holder will have a dual role combining both clinical and corporate leadership responsibilities, and work within Heeley Plus Network policies and procedures. The post holder is expected to deliver clinical care to patients, relevant to their professional skill set.

The post holder will provide professional specialist advice to the network leadership team on the development of the Medical Health professional team.

The post holder will also take a lead role in the development of programmes which support the training and deployment of members of the allied health professional team and make recommendations for local planning and service development.

Lead Responsibilities

The Lead will be responsible for the development of the Medical Support team model.

This will include;

· Working with the Clinicians, to ensure robust governance arrangements are implemented.

· Developing effective communication with the Clinical Director, Network manager and member practices to ensure meeting of organisational and national standards and requirements.

· Work closely with the other team leaders in the network to ensure a joined up and complementary approach across teams.

· Providing appropriate mentorship.

· Ensuring access to continued professional development opportunities.

· Advise on the development of job plans for other the multidisciplinary team members (Eg Physician associates, Occupational Therapists, physiotherapists, paramedics, nurse associates, practice nurses, advanced care practioners) to ensure they reflect both service need and the person’s competencies and skills.

· Ensure a co-ordinated approach to service delivery within the medical support team, appropriately distributed across the member practices

· Leading on the development and implementation of protocols and procedures which direct and inform the staff in your team.

· Raise and support the management of fitness to practice issues and any other concerns.

· Lead the recruitment and retention of a range of roles that support holistic medical management of patients

· Ensuring that expenditure is contained within the allocated budget, and to develop and monitor a fully costed financial plan to support the development of this role.

The Holder of this post will offer clinical services inline with their skill set and qualifications, as agreed by the PCN leadership team. They will act within a predetermined level of supervision and within agreed national guidelines.

They will be aware of the limits of their competence and committed to acting within these limits.

They will be expected to maintain their broad clinical knowledge base through continuing professional development (CPD) and will be required to sit for a national re-certification as appropriate to their speciality.

An annual appraisal and quarterly reviews will be carried out with the Lead Clinician and/or Network Manager or nominated representatives.
