Job summary
Moreton Group Practice is a large medium sized Practice with 13250 patient list size. We have pleasure of serving patients living in the Moreton area.
An exciting opportunity has arisen within our HCA team due. You will be joining a group of HCA's who all supported our 12 GP's, 3 ANP's and 5 nurses. You will be providing support to all clinical staff as well as holding your own clinic session.
Main duties of the job
The post holder works
as part of the Nursing team under the supervision of registered nurses and lead
HCA.
Participates in the delivery of patient centred care, always maintaining a
compassionate and professional attitude.
Contributes to the practice organisation.
Support registered nurses in the provision of a caring and safe environment.
The post holder will demonstrate competence in generic and
specialist skills as required within a time frame agreed with their manager
following the completion of appropriate training and education. The Health Care
Assistant will function at a competency level in line with the requirements of
the post and achieve and maintain both mandatory and statutory training
requirements associated with the role.
KEY WORKING RELATIONSHIPS:
All members of the
multidisciplinary team, including nursing and medical staff, allied health care
professionals, support workers and administration and clerical staff.

Here atMoreton Group Practice we have a clinical team consisting of GPs, four ANP's, four practice nurses and three HCA's. They are supported by a large admin team comprising of Medical Secretaries, Medicines Managers and Administrators.
We were rated "Good" at our last CQC inspection. We are high achievers in QOF and other enhanced services.
Moreton Group Practice are also attached to Moreton and Meols PCN who provide additional health professional roles, for instance pharmacists, physiotherapists, paramedics, health & well being coaches and more to our team.

part of the multi-disciplinary team supporting colleagues in work associated
with patient care
Understand own level of
responsibility and accountability in relation to registered staff.
Communicate clearly, effectively and appropriately with the multidisciplinary
team, patients and their family, visitors and carers.
Through practice demonstrate an understanding of Duty of Care and how this
contributes to safe practices.
Assist and as required provide patients with personal care
Following appropriate training and supervised practice, undertake identified competency-based
practices which are required for the Practice, after such tasks have been
delegated to them by a registered nurse or manager.
Identify learning and
development needs and produce a personal development plan in conjunction with
your line manager / supervisor.
Achieve and maintain competence in mandatory and statutory training associated
with the role.
Complete documentation of
care in line with the Practice Standard.

- Experience working within either a General Practice or hospital environment is essential.
- Ideally you will have the following experience and be fully able to conduct the following.
- Annual Review Workup for Chronis Disease
- 2. Diabetic Foot Checks
- 3. Hypertension Monitoring
- 4. Learning Disability Annual Health Check with ANP's
- 5. NHS Healthchecks
- 6. New Patient Health Checks
- 7. Lifestyle Advice/Weight Management
- 8. INR Monitoring
- 9. Vaccine Administering - b12/flu/Pneumonia & Shingles
- 10. Urinalysis/Sample Collection
- 11. Home Visits, you must possess a full drivers license.
- 12. Phlebotomy when needed as we have an inhouse service

- You must be an experienced HCA, ideally from General Practice however training can be given to the right candidate.
