Job summary

The ideal candidate should have a good understanding of Primary Care. Qualified in Chronic Disease Management, Immunisation and Cytology or currently undertaking these qualifications. They should have an understanding of Qof and be committed to providing excellent care to our practice and PCN population

Main duties of the job

To work as a team providing effective care to our practice and PCN population accordance with the agreed protocols and objectives

To assess, plan, deliver and evaluate care holistically to the practice population

Clinical Support on the management and care of patients with Long Term Conditions

Plan, implement and evaluate individual treatment plans, within the remit of your own role

About us

We are a 3 Partner, 4 Salaried GP, 3 ANP and 3 PN team supported by 1 HCAs and 1 TNA and a full Management and Admin team. We are situated next door to Sunderland Hospital in a purpose built building. We work together to provide the best service possible to our 10,300 patients from across the city. We are part of a 7 Practice PCN in Sunderland West providing services at scale including Enhanced Access.
