Job summary
Greystones Medical Centre have a vacancy for a part-time Advanced Nurse Practitioner (ANP) to join our
friendly practice. The candidate will
ideally have relevant experience within a primary care setting.
Main duties of the job
The post holder will provide care for the
presenting patient from initial history taking, clinical assessment, diagnosis
and treatment whilst demonstrating safe, clinician decision making and expert
care.
To work collaboratively with the team to meet
the needs of the patient.
Refer patients to other medical specialities for
assessment if appropriate.
Diagnose and manage acute and chronic
conditions.
Advise and educate patients promoting
understanding, self management and healthy lifestyle choices.
Contribute to practice targets within disease
management for QOF and IIF and support the delivery of policies and procedures.
Ensure accurate records are maintained with all
patient encounters.
