Job summary

The Lead Practice Nurse will provide senior clinical leadership across the nursing and HCA team, ensuring high quality, evidence based care and effective service delivery. This role oversees the development, supervision, and mentorship of new and existing clinicians, while strengthening collaboration with community, secondary, and social care partners.

The postholder will lead key public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion, and cervical screening, and will embed population health approaches that address inequalities and improve access.

As a core member of the practice leadership team, they will contribute to clinical policy, support multidisciplinary working, and ensure robust systems are in place for the management of long term conditions, including care for elderly and housebound patients. This position offers the opportunity to shape nursing services, influence strategic priorities, and play a central role in delivering safe, integrated, and patient centered primary care.

Main duties of the job

The Lead Practice Nurse will deliver core clinical and leadership responsibilities of the nursing team, acting as the Infection Prevention and Control lead, completing audits, training, and compliance reporting, and producing the annual IPC statement with the Practice Manager. They lead longterm condition management, support public health and screening programmes such as immunisations and cervical screening, and provide evidencebased routine nursing care aligned with NICE and NSF guidance. The role includes mentoring and supervising newtopractice nurses, supporting the wider nursing and multidisciplinary teams, and contributing to clinical audits, research, and the development of local protocols.

The postholder works within national and local guidelines, recognises professional boundaries, and ensures appropriate referrals. They supervise student nurse associates, uphold safeguarding responsibilities, complete mandatory training, and contribute to clinical governance and practice targets. Accurate clinical records, effective use of SNOMED CT, maintenance of chronic disease registers, and delivery of call/recall systems are essential. They also chaperone patients when required, support clinical emergencies, liaise with external agencies for vulnerable patients, and provide opportunistic health promotion.

Job responsibilities

The Lead Practice Nurse will be responsible for the management of the practice nursing team, consisting of nurses and HCAs.
