Job summary

The role will support GP practices across the Southport and Formby Primary Care Network (PCN) area to reduce patient demand and increase/improve outcomes for patients, focusing on specific key themes explained further below. The role will become an integral part of the practice team and will be supported by the wider practice team. The individual will be expected to attend training sessions, team meetings and any support opportunities made available to you by the GP Practice.

The individual must be happy to work with both clinical and non-clinical colleagues, both internally and externally from the practice to best support patients and practices generally. The individual must have experience of using clinical systems such as EMIS and be competent in the use of Microsoft Office.

Main duties of the job

Manage Cancer Registers - Organise cancer care reviews with lead Clinicians, ensure cancer care reviews are completed and coded correctly and complete referrals to Cancer Navigators.

Management of Cancer safety-netting templates.

Manage Gold Standard Framework (GSF) registers Identify patients, manage register, organise and minute quarterly GSF meetings and complete referrals as necessary.

Manage Learning Disability (LD) registers - Arranging LD reviews with Practice Clinicians, ensure Action Plans are completed and shared, manage Action Plan review meetings and use Team Net LD resources to comply with LQC requirements.

Manage (SMI) registers - Arrange blood tests and clinical reviews, ensure reviews are completed and coded correctly using the correct templates, identify hard to reach patients and discuss with Clinicians on how best to manage the reviews. Refer to hard to reach service where appropriate.Manage Long Term Conditions (LTC) reviews in practice - Ensuring all patients eligible have review on time, reviews are coded properly, and appropriate referrals are completed. Highlight coding issues and performance at quarterly practice meetings.

Manage Housebound register Ensure records are up to date and accurate.

Support recall systems for Local Quality Contract.

Manage ICT MDT within practice Create an appropriate referral process, arrange a system for Clinicians to dial into meeting and ensuring actions are recorded and noted in clinical records.

About us

Norwood Surgery, located at 11 Norwood Avenue, Southport, Merseyside, PR9 7EG, is a General Practice serving approximately 9,800 patients.

The practice is led by five GP partners and supported by salaried GPs, sessional GPs, and GP registrars. An Advanced Nurse Practitioner is available to assist with minor ailments and can prescribe and refer as necessary. The nursing team comprises three practice nurses who handle a variety of appointments, including annual reviews of long-term health conditions, diagnostic procedures, and contraceptive care. The practice operates Monday to Friday from 8:00 AM to 6:30 PM.
