South Leicestershire Medical Group are looking for an experienced practice nurse to join our team. We are a friendly, supportive, and forward thinking partnership committed to delivering high-quality primary care to our local community. Our team includes GP's, ANP's clinical pharmacists, HCAs and a dedicated patient services, admin and secretary team who work collaboratively to provide outstanding patient care. We are now looking for an enthusiastic and compassionate practice nurse to join our multidisciplinary team.

Main duties of the job

The successful candidate will work as part of a multidisciplinary team to provide high-quality nursing care, including treatment room services such as wound management, immunisations, cervical screening and health checks. Supporting long-term condition management, asthma, COPD, diabetes, cardiovascular diseases. Provide health promotion, lifestyle advise and disease prevention information. Work autonomously within the clinical team to assess, plan and evaluate patient care. Maintain accurate, up to date clinical records and contribute to practice targets, quality frameworks and patient safety processes.

Job responsibilities

The successful candidate will provide comprehensive nursing services to support the effective delivery of primary care. Working both independently and as apart of a multidisciplinary team, the post-holder will undertake a wide range of treatment room and chronic disease management duties. They will assess patient needs, plan and implement care and evaluate outcomes in accordance with evidence-based practice and clinical guidelines.

Key aspects of the role include managing long-term conditions such as asthma, COPD, diabetes, hypertension and cardiovascular disease. This involves conducting reviews, supporting self management, monitoring treatment effectiveness, adjusting care plans under clinical protocols and collaborating with GPs and other clinicians where needed.

Routine treatment room responsibilities from a core part of the role and include wound care, dressings, injections, vaccinations (childhood, adult, travel), cervical cytology, ECGs, blood pressure monitoring, ear care and assisting with minor procedures. The practice nurse will also provide contraceptive and sexual health services and general health promotion activities aimed at improving patient wellbeing.

The role contributes to key practice performance requirements, such as QOF, IIF and safeguarding processes, ensuring high standards of clinical compliance, infection control and patient safety. The post holder will maintain detailed clinical records, support continuous improvement and participate in audits, training and clinical meetings.

Strong communication, clinical reasoning and organisations skills are essential, as the nurse will often manage their own clinics, prioritise workload independently and work closely with GPs, ANPs, Pharmacists, HCAs and administrative colleagues.
