Community GP South Leicestershire Medical Group

South Leicestershire Medical Group is seeking a dedicated and compassionate Community GP to join our progressive, supportive, and forward-thinking general practice team.

Main duties of the job

This unique role focuses on delivering high-quality, person-centred care to care home residents, housebound patients, and individuals living with frailty within our community. You will work alongside our existing Community GP and two Practice Community Nurses as part of a close-knit, multidisciplinary team dedicated to improving outcomes for some of our most vulnerable patients.

Job responsibilities

As a key part of our community service, you will:

  • Provide routine and urgent consultations for care home residents, housebound patients and frail or elderly individuals
  • Complete comprehensive care planning
  • Manage long-term conditions within community patients
  • Work closely with our multidisciplinary team including nursing, therapy, pharmacy and social care professionals
  • Support palliative and end-of-life care planning
  • Collaborate with care homes and families to provide coordinated care.

  • Commitment to continuity and holistic care.
  • Experience or willingness to work in community settings.
  • Strong communication and documentation skills.

  • Experience in frailty and community care.
