Job summary

We are currently looking to recruit a clinical pharmacist to work Tuesday, Wednesday and Friday each week. The post holder will be joining our existing pharmacist and PCN pharmacist team.

This is a key role within our multi-disciplinary practice team and we anticipate the role will evolve with the continued professional development of the post holder.

This position will be both patient facing and supporting the practice team.

Main duties of the job

Working as part of the practice team this will be a varied role which will include supporting the administration of prescriptions by the reception team and dealing with patient and community pharmacy prescription queries. Reviewing discharge summary medications, supporting the PCN pharmacy team with high risk drug monitoring and structured medication reviews.

For candidates who are working towards, or already hold an independent prescribing qualification, having hypertension and/or diabetic review clinics.

Reviewing CQC prescribing searches and actioning.

We have a clinical supervision system and the clinical team meets weekly for case based discussions and reflective learning. We are actively engaged with our One Thornton Heath PCN and with our Croydon North East Local Community Partnership. We are increasing our network links with other health related businesses in the community.

Job responsibilities

Main Responsibilities

Undertake structured medication reviews with patients, including care home patients. Working with patients to support safety, reduce waste and promote self care

Be responsible for the medicine optimisation of patients with one or more long term conditions. Reviewing the ongoing need for each medicine, review monitoring needs and proactively managing patients with complex polypharmacy

To receive and resolve medicine related queries from the practice team, patients and community pharmacists

Reconcile medicines following patient discharge or attendance at hospital appointment or admission

Manage the repeat prescribing process by implementing a repeat prescribing policy and managing and reviewing patients requests for repeat prescriptions. Ensuring patients have the required monitoring tests

Agree and review prescribing formularies and protocols and monitor compliance levels

Signpost patients to the appropriate healthcare professional for the necessary level of care within the appropriate time frame

Identify and monitor patients on high risk drugs

Review medications for newly registered patients

Improve the quality and effectiveness of prescribing through clinical audit and education

Act upon and implement changes to medicines that result from MHRA alerts, NICE guidance, product withdrawal and other local and national guidelines

Work with the practice team to ensure the practice is compliant with CQC medicines standards. Running monthly searches to ensure compliance

Support the delivery of QOF, IIF, local and national incentive schemes, QIPP and other quality or cost effectiveness initiatives

Deliver training, mentoring and guidance to other clinicians and staff on medicine issues

Work collaboratively with the practice team, and colleagues in the PCN, ICB, secondary care and community pharmacy

Contribute to public health campaigns
