We have an amazing job opportunity for a Practice Nurse to join our award winning, forward thinking and growing team on a full or part time basis. As a Practice Nurse you will be supported by the partners, Senior Management Team and a dedicated team. You will work collaboratively with the wider team and organisation to meet the needs of patients and provide leadership and direction for the nursing team. Our ideal candidate will have drive, dedication, enthusiasm, and a positive attitude. They will have the ability to work well with all teams across all our sites and be open to innovative ways of working, including working collaboratively with our 20 + nursing colleagues working across our sites.
Main duties of the job
The key focuses of the role will be supporting the wider practice team in delivering care to our residents. This includes monitoring and assisting in long-term conditions management and proactively supporting health promotion and screening activities.
Skills in immunisation, and cervical smear provision are essential requirements.
The successful candidate will be expected to:
- Provide general nursing care including, adult and child immunisations, travel vaccinations, cervical smears, wound care and chronic disease management etc in accordance with guidelines
- Maintain nursing policies and procedures.
- Ensure self-development and registration requirements are kept up to date and regularly reviewed.

- KEY RESPONSIBILITIES
- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
- To develop, implement and embed health promotion and wellbeing programmes
- To work in conjunction with the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing evidence of compliance where necessary. To assist the IPC lead in producing the IPC annual statement
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
- To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
- To provide routine nursing care (including cervical screening, childhood immunisation and learning disability checks) to patients in accordance with practice policies and protocols
- To support the provision of travel vaccination services
- To ensure accurate, clinical details are recorded at all times
- To ensure compliance with the practice chaperone policy
- To ensure CQC Compliance is met at all times
- To support the team in dealing with clinical emergencies
- To implement vaccination programmes for adults and children
- To deliver opportunistic health promotion where appropriate
Provide education to patients managing chronic conditions including healthy lifestyle changes and preventative care
- Maintain accurate and up-to-date patient records
- Offer emotional and psychological support to patients and safeguard where necessary
