Main duties of the job
Deliver high-quality, patient-centred care at all times.Undertake consultations in surgery, via telephone, and home visits as required.Collaborate with colleagues and external providers to ensure seamless, integrated care.Investigate, diagnose, and refer patients appropriately in line with clinical guidelines.Review test results and correspondence promptly and accurately.Provide cover for colleagues when necessary to maintain service continuity.Collect, analyse, and review data for audits and QOF compliance.Maintain clear, compassionate communication with patients and their carers.Participate in clinical meetings and contribute to team discussions.Work collaboratively within the multidisciplinary team to achieve shared goals.Manage your own workload and resources effectively and efficiently.Attend mandatory training sessions and provide feedback when required.

About us
Our team includes 5 experienced GP Partners and currently 3 Salaried GPs, supported by a multidisciplinary team of Advanced Nurse Practitioners, Practice Nurses, HCA's, GPA's and skilled administrative staff.
As a teaching practice, we proudly support medical students from Hull York Medical School (HYMS) and the University of Lincoln, fostering the next generation of healthcare professionals. Job responsibilities
- Conduct face-to-face, telephone, and video consultations, managing a wide range of acute and chronic medical conditions.
Provide timely and appropriate prescribing, referrals, and follow-up care.
Perform clinical examinations, interpret diagnostic tests, and formulate treatment plans.
Deliver proactive care through health promotion, disease prevention, and lifestyle advice.
Maintain accurate, comprehensive, and timely clinical records using SystmOne
Ensure documentation meets medico-legal standards and supports continuity of care.
Complete reports, forms, and letters as required for patient care, insurance, safeguarding, or legal purposes.
Work closely with practice nurses, healthcare assistants, pharmacists, social prescribers, and other allied health professionals.
Participate in multidisciplinary team (MDT) meetings to discuss complex cases and coordinate care.
Liaise with secondary care, community services, and social care teams to ensure integrated patient management.
Contribute to Quality and Outcomes Framework (QOF), Enhanced Services, and other performance targets.
Participate in clinical audits, significant event reviews, and quality improvement initiatives.
Support the practices compliance with CQC standards and clinical governance policies.
Identify and act on safeguarding concerns for children and vulnerable adults.
Follow practice protocols for risk assessment, incident reporting, and escalation.
Maintain up-to-date knowledge of safeguarding legislation and best practice.
Engage in continuing professional development (CPD) and maintain revalidation requirements.
Attend practice meetings, clinical education sessions, and peer reviews.
Participate in duty doctor rota and on-call responsibilities as required.
