ACCOUNTABLE TO:Clinical Director Dr T Douglas (Clinically)

PCN Manager Katie Smith (Administratively)

HOSTED BY: Rother Valley South Primary Care Network

PLACE OF WORK: Rother Valley South PCN GP Practices

An exciting opportunity has arisen to join our multi-disciplinary PCN team.

We are a dynamic, forward thinking PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

The PCN has been successful in becoming part of a national pilot scheme which will look at ways to make General Practice Sustainable. The PCN is mobilising a Home Visiting team to support the practices with home visits.

We are therefore looking to expand our team by employing Advanced Clinical Practitioners to work alongside our practices and PCN staff.

Our PCN staff currently includes Clinical Pharmacists, Physicians Associates, Social Prescribers, Paramedics, Physiotherapists, Care Coordinators, Nurse Associates and Pharmacist Technicians.

Main duties of the job

The Advanced Clinical Practitioner(ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals (HCP) setting and job role.

The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment within their own homes (where appropriate).

An ACP is autonomous in making decisions based on assessment, diagnosis and interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

The post holder will need to be capable of independent travel across the Rother Valley South area to provide home visits and work flexibly to meet the needs of the Business.
