Job summary
Chorley Central Primary Care Network are looking for an experienced Care co-ordinator to join the PCN Care Home team.
You will be part of an established team delivering the Enhanced Care in Care Home (EHCH) service for the 16 Care Homes in the PCN.
We are looking for someone who is passionate about
delivering person-centred care, has a professional approach to their work, who
is caring, dedicated and compassionate to the needs of the patient.
Main duties of the job
This role will be based at Regent House Surgery and will involve traveling between care homes.
Working as part of a well established team and
strictly in accordance with PCN EHCH Guidelines and protocols.
Conducting 6 monthly health and care plan reviews.
Ensuring all residents have a Personalised Care and Support Plan (PCSP)
Support team in administering Influenza and COVID vaccinations.
Administration support for the weekly MDT meetings
Hours are 9am -2pm Monday - Thursday

Knowledge
Essential
- Understanding of systems to gain an understanding of the health needs of the population as they relate to primary care
- Understanding of evidence based practice
- Knowledge of national standards that inform practice (e.g. National Service Frameworks, NICE guidelines etc)
Desirable
- Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework
Qualifications
Essential
- Experience of working in health, social care and other support roles in direct contact with people, families and carers.
- Experience in performing and recording basic health measures i.e. blood pressure, oxygen saturation, respiratory rate, weight etc.
- Experience of dealing with confidential and sensitive matters.
- Experience of working independently and organising and prioritising own workload.
- Excellent written and verbal communication skills as well as having strong time and case management abilities.
- Car driver / access to a car
Desirable
- NVQ Level 2 in Health and Social Care or equivalent.
- Experience of working with older, vulnerable patients or adults with complex needs
- Experience of EMIS or other medical software
