Job summary

Chorley Central Primary Care Network are looking for an experienced registered nurse to join the PCN Care Home team.

You will be part of an established team delivering the Enhanced Care in Care Home (EHCH) service for the 16 Care Homes in the PCN.

Main duties of the job

completing six monthly care plan assessments

chronic disease reviews to meet with QOF requirements

phlebotomy when HCA on leave

flu vaccinations

To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

Refer patients to secondary/specialist care as required

To maintain accurate clinical records in conjunction with extant legislation

Experience

Essential

  • Ability to assess and manage patient risk effectively and safely
  • Time management and ability to prioritise workload
  • Able to analyse data and information, drawing out implications for the individual patient/impact on care plan
  • Able to establish and maintain effective communication pathways within the organisation.
  • Excellent interpersonal, verbal and written communication skill
  • Car driver/access to car

Desirable

  • Experience of using a medical software package

Knowledge

Essential

  • Understanding of systems to gain an understanding of the health needs of the Practice population as they relate to primary care
  • Understanding of evidence based practice
  • Knowledge of national standards that inform practice (e.g. National Service Frameworks, NICE guidelines etc)

Desirable

  • Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework

Qualifications

  • Registered General Nurse (Currently registered with the Nursing & Midwifery Council)

  • DNACPR Policies Procedures and Documentation
