Job summary

Due to the current post holder moving into a new role within the team, Folkestone Hythe and Rural PCN are recruiting a new PCN Care Co-ordinator. The post holder will be an integral part of the PCN team and working closely with clinicians. They will also work closely the PCN Senior Team, Clinical Director and Practice Managers to ensure efficient operation of PCN services. Reporting directly to the PCN Operational Manager.

The PCN have utilised considerable resource and expertise to ensure all digital systems are in place to support clinicians and administrative staff to work seamlessly across our member practices. The successful applicants will be given the opportunity to refine and continually evaluate the new operating model in order to support iterative development.

The post holder will have excellent communication skills and be flexible in their approach, using their initiative to manage conflicting priorities. The right candidate will be confident in using MS Office (word, Excel, Power-point and Outlook) and in learning to use new computer systems. Experience in general practice is desirable but not essential.

Main duties of the job

The post holder will play a key role at the heart of our GP practices, working alongside other PCN colleagues to deliver holistic and coordinated care to our patient community. They will be have a defined work-plan, managing projects and co-ordinating other contractual requirements, they will also supporting the achievement of key health and welfare objectives of Folkestone, Hythe and Rural PCN.

This role is designed to ensure new pathways and/or services are coordinated efficiently and effectively, ensuring robust communication with all stakeholders.

Help people to manage their care needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information relating to their care.

All of our member practices are fully committed to supporting the development and implementation of PCN level services. Services are delivered within several of our practices and fully supported by a GP's, Practice Managers and PCN Senior Team.

This role is designed to support the delivery of PCN services, co-ordinating and providing non-clinical triage for patients, also providing robust communication with patients, and their families when necessary. Working as a key part of the PCN Team.

The post holder will work alongside clinicians and other staff over the seven practice sites and as part of a multi-disciplinary team. They will take responsibility for an area of work that will contribute to fulfilling the requirements of the Network Contract Directed Enhanced Service (PCN DES).

Provide a point of contact for both staff and patients, in coordinating care and will help to deal with incoming queries relating to PCN level pathways or processes.

Help people to manage their care needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information relating to their care.

Arrange appointments for patients on the PCNs case load, or otherwise, as directed by clinicians, following identification or urgent and non-urgent clinical need, to assess diagnose and treat according to the patients health needs.

Post holders will need to demonstrate flexibility and adaptability to working in a dynamic and busy environment.

Refer patients through to the appropriate team member, and/or make referrals on behalf of the team.

Signpost patients to appropriate community or local services.

Serve as the contact point, for practice staff, providers, care teams, family/caregivers and community resources where indicated, responding with empathy and respect.

Support the PCN to deliver on quality metrics, such as QOF, KPIs and locally commissioned enhanced services.
