Job summary
Wyre Forest Health Partnership (WFHP) is a large, forward-thinking GP Partnership serving approximately 73,000 patients across five practices in the Wyre Forest area.
We are seeking a motivated and enthusiastic Clinical Pharmacist to support the Partnership in implementing effective medicines management, identifying areas for improvement, and initiating change in collaboration with the senior clinical team.
With practices situated in Kidderminster, Bewdley, Stourport and Hagley, WFHP serves both urban and semi-rural communities. The successful applicant will be primarily based at Kidderminster Medical Centre, with cross-site working both in person and virtually where required.
We offer flexible working arrangements, including consideration of up to one day per week working from home. All pharmacists are provided with laptops to support flexible and remote working.
This is an exciting opportunity to join a supportive pharmacy team of 8 Clinical Pharmacists and 8 Pharmacy Technicians delivering high-quality, patient-centred care.
We welcome applications from pharmacists with primary care experience, as well as those looking to transition into primary care. We support completion of the CPPE Primary Care Pathway and welcome newly qualified pharmacists. As a training practice for foundation pharmacists, we offer strong support for career development and progression.
Main duties of the job
The Clinical Pharmacist will support the delivery of high-quality, patient-centred care through effective medicines optimisation and clinical services. Key responsibilities include managing medicines following hospital discharge, conducting medication reviews including Structured Medication Reviews, supporting repeat prescribing, and providing remote consultations.
The role also includes supporting education and training across the practice, providing medicines information, supporting pharmacy staff development, and participating in regular CPD and team learning sessions.
