Job summary

We have an exciting opportunity for a Care Coordinator to liaise with patients and relatives to ensure that patients have access to the most appropriate care environment for their own specific needs.

This post will provide an opportunity for individuals with excellent communication skills to act as role models and be part of developing new ways of working and support the delivery of patient care. The care co-ordinator will be responsible for contacting patients, helping them to navigate their healthcare and directing them to the most appropriate service for suitable care.

This role could see the successful applicant supporting in the co-ordination of care in various different settings, including Asthma, Vulnerable Adults and Mental Health.

Main duties of the job

Be a key contact for patients with health conditions, helping them to understand and manage their health and wellbeing.

Co-ordinate patient healthcare, directing to the most appropriate service for suitable care.

Work across multi-disciplinary teams of healthcare providers (Such as Pharmacists, GPs, Nurses)

Job responsibilities

Be a key contact for patients with health conditions, helping them to understand and manage their health and wellbeing.

Co-ordinate patient healthcare, directing to the most appropriate service for suitable care.

Work across multi-disciplinary teams of healthcare providers (Such as Pharmacists, GPs, Nurses)

Work with GPs and other primary care professionals to identify, manage and prioritise a caseload of patients in accordance with needs and, where required and as appropriate, refer people back to other health professionals within the PCN (Primary Care Network).

Help patients to manage their care needs, serve as a point of contact, advocate and information resource for patients, family and caregivers.

Act as a building block for care by co-operating with other service providers to share information, jointly plan care and provide a streamlined, integrated service and to reduce variations in service delivery in adherence with confidentiality and data protection legislation and data sharing agreements.

Work with the PCN team to increase uptake of healthcare educational courses, including proactively recruiting and referring through direct patient contact.

Ensure regular and consistent communication with the referrer regarding patient progress and any complications or guidance.

Have mechanisms in place to engage with patients who are using the service and their carers.

Ensure at all times, service users experience support and contact that is user friendly, accessible, responsive to individual needs, warm and professional.

Support the PCN to reduce inequalities by targeting cohorts of patients who struggle to engage with healthcare services.

Build positive professional working relationships with other professionals, both clinical and non-clinical across the PCN.

Experience
