Job summary

Torquay Primary Care Network was formed in 2019 following the launch of the national Primary Care Network DES. In addition to the DES, some services and funding are contracted through the PCN which also places a responsibility for all Practices to collectively optimise service delivery and targets. Good financial control is needed to maximise efficiencies and opportunities. TPCN works with two other PCNs across Torbay under the umbrella of Torbay General Practice, and collaboratively deliver services such as Extended Access, The COVID Vaccination Programme and the Torbay Care Home Visiting Service.

There are contractual drivers to encourage practices to work together as PCNs, but TPCN also feel that it is important that Practices are able to maintain their identities and be respected in decision making and communication. We have seen great benefits in sharing good practice, achieving projects, recruiting and sharing staff, and more recently in supporting each other during the coronavirus outbreak and vaccination programme. The PCN is now looking for an efficient and effective PCN Manager to work closely with the Clinical Director, PCN Board and member Practice Managers to meet the requirements of the DES, maximise service delivery and financial efficiencies, recruit shared team members, maintain good communication and provide good financial reporting.

Main duties of the job

  • Help develop strategies to move the PCN along the Maturity Matrix over the coming years.
  • Promote a culture which encourages teamwork, sharing of best practice/education and continuous improvement.
  • Foster good communications and relationships with and between member Practices, as well as other colleagues.
  • Assist and advise on the management and delivery of the PCN DES, CCG contracts and any other contracts or requirements.
  • Report and advise on achieving objectives and targets.
  • Maintain good financial control and report accordingly to the Clinical Director and PCN Board on finances and cash flow. Liaise with the Finance Manager to ensure that payments to practices are made in a timely manner.
  • Ensure that the PCN gets best value and maximises efficiencies with regard to its finances and commitments.

  • Work with other health and social care colleagues, as well as the voluntary sector, in order to provide the best possible patient care within an integrated care system.
  • Co-ordinate and service PCN meetings and events, including setting dates, preparing agendas, taking notes, maintaining an action plan and communicating decisions to the wider PCN.
  • Keep abreast of national and local information in order to inform and advise members of the Network.
  • Work with patient representatives and communicate with patient groups as appropriate.
  • Represent the PCN in a professional and informed manner.

ORGANISATIONAL:

  • Work closely with the Practice management team to develop and support the aims of the PCN and make efficiencies in ways of working.
  • Ensure that the PCN has appropriate corporate policies and procedures in place. This will include developing policies for Practices to jointly adopt, e.g. Business Continuity and IM&T.
WORKING RELATIONSHIPS:
  • Provide leadership and develop this role as the PCN evolves in its functions and responsibilities.
  • Develop ways of working to improve patient care across an integrated care system with other providers by using data available to analyse health population needs.
  • Take an active role in the development and embedding of the PCNs culture, values and reputation as providers of high quality services as well as being good employers.
  • Encourage staff to collaborate through sharing information and intelligence across different activities.
  • Organise PCN events e.g. educational and strategic events.

OTHER:

Personal/Professional Development:

The post-holder will participate in mandatory training and any training programme implemented by the PCN as part of this employment, such training to include:

  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
  • Taking responsibility for own development, learning and performance and demonstrating skill and activities to others who are undertaking similar work
  • Training may need to be undertaken outside of normal practice hours, and off site.
  • Attend and participate in quarterly protected time learning events and staff meetings when required.
