Modality Partnership
has a vacancy for a new and developing role, Inclusion Health Manager. The postholder will be a high performing
manager working to support socially isolated people across Modality Hull, and
across the city to address health inequalities and ensuring patients receive
personalised, high quality care. We are
seeking dedicated postholders to join our team and who can display our CARE
values: Commitment, Accountability, Respect and Excellence.
This job is
suitable for individuals who are career driven and have a passion for working in
an agile manner to deliver priorities that address health inequalities, and who
are committed to make real changes to the health care services provided within
Primary Care to improve patient care. People who have been socially
excluded are more likely to have multiple long-term conditions, have poor
health outcomes, struggle to access mainstream healthcare and ultimately have
shorter life expectancies than the general population.
This job is not
suitable to people who are not resilient to working in a fast-paced environment,
working flexibly in line with ever changing demands in Primary Care and the
wider NHS.
This is a part time vacancy and the salary is based on whole time equivelant.

Main duties of the job
The role is an operational
management role which requires the postholder to have responsibility for our
Homeless Health Service and other health inclusion
provision, providing primary care for homeless patients in the hospital and
community setting. You will drive all
elements of the service, developing operational and strategic partnerships
across the sector managing the service and the team, and ensuring the contract
delivers excellent care for the homeless people in Hull.
The postholder
will work in collaboration with partners across the city to ensure patients can access a seamless pathway of care, ensuring that contact with
the hospital is used as an opportunity to address multiple health needs.
The role is not
a regular 9am-5pm job and is not a pure management role; the job requires innovation,
flexibility, and commitment and the postholder will be required to work
resourcefully as part of the team to ensure tasks are completed.
