Job summary

We are excited to be recruiting to the position of Care Navigator.

The Care Navigator function requires you to interact with patients to ensure they are offered a positive experience by providing professional front line and call handling services.

This Care Navigation role will be cross site working and the successful candidates will be required to cover across any of our four Hull based GP practices, Newland Group Practice, New Hall Surgery, Springhead Medical Practice, and Diadem Medical Practice. We are recruiting to a range of hours, of which both part time and full time hours are available. There will be an element of early evening work included within the shifts available as well as occasional weekend and bank holidays as and when required.

Main duties of the job

The Care Navigator will interact with patients to provide and process information in response to enquiries, concerns and requests about NHS/practice services. They will ensure that all patients who access our sites whether it is face to face or via telephone have a professional and positive experience. The Care Navigator will work as part of the reception team in a professional manner to provide high quality reception, administration and support services to all clients, visitors, GPs and Allied Health Professionals.
