Main duties of the job
As a Care Navigator, you will be at the heart of patient caremanaging appointments, handling queries, and guiding individuals to the right services both within and outside the practice. You will also communicate with doctors, nurses, and healthcare professionals, as well as undertaking some administrative duties, ensuring smooth day-to-day operations. This role is ideal for someone who thrives in a busy environment and values helping others.

Job Summary Care Navigator
As a Care Navigator at Cornerstone Practice, your main responsibilities will include:

Welcoming patients in person and over the phone, offering a calm, friendly, and professional first point of contact.

Navigating patients to the most appropriate healthcare professional or service, using clinical protocols and your knowledge of local services.

Managing appointments via our computerised system, ensuring efficient scheduling with doctors, nurses, and other staff.

Handling patient queries, prescription requests, test results, and other health-related enquiries with tact, accuracy, and confidentiality.

Supporting clinical teams with administrative duties, including maintaining accurate patient records, preparing clinic documents, and processing incoming correspondence.

Responding to urgent situations, including arranging home visits or emergency services when needed, following agreed procedures.

Liaising with external agencies, such as hospitals and community services, to support continuity of care.

Maintaining a welcoming environment, ensuring patient areas are clean and ready for use, and that the building is opened and closed safely.
