Job summary

Cornerstone Practice is seeking Advanced Practitioners to come and join our thriving team as we expand our multi-disciplinary team to provide whole person care to our large patient population.

You will be responsible for the clinical assessment, treatment and/or referral or giving advice as appropriate to patients presenting with a wide spectrum of clinical conditions. You will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Were interested to hear from enthusiastic Advanced Practitioners from any clinical background including Nursing, Paramedic, Physiotherapy, Pharmacy or other roles that might serve general practice well.

Main duties of the job

You will workwithin your professional boundaries, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. You will demonstrate critical thinking in the clinical decision-making process and hold a Non-Medical Prescribing qualification.

Working collaboratively with the general practice team to meet the needs of patients and supporting the delivery of policy and procedures will be a daily requirement.

Patient care will be provided face to face, online, via telephone and through home visits as appropriate.

We are an accredited training practice,training and supporting GP Registrars, ACPs, Practice Nurses and Clinical Pharmacists. We serve 26,500 patients and we particularly want to care for those living in deprived communities.

Blackburn is situated in a beautiful part of Lancashire, and our surgeries are all easily accessible from the M65.
