Due to retirement, we have a fantastic opportunity for an Advanced Nurse Practitioner to join our team at Adur Health Partnership (AHP).

Candidates will ideally have extensive experience in acute / primary care nursing and be specifically competent and confident in minor illness presentations and management. Candidates will work as an autonomous practitioner, but working as part of the practice multidisciplinary team. They will hold an ANP qualification with up to date NMC registration, plus evidence of continual professional development.

The successful candidate will be based at one of our Partnership sites within Shoreham and Southwick but expected to travel between the practices as required.

Main duties of the job

To work as an autonomous practitioner, to assess, diagnose and manage patients

with minor illnesses and injuries, including prescribing medication, whilst also

identifying where the issue is more complex, and collaborating with other healthcare

professionals accordingly.

To work alongside the GP and practice multidisciplinary team, delivering care within

scope of practice to the patient population supporting the Management and

Executive teams in the reviewing of clinical policy and procedure.

Job responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may

be on occasion, a requirement to carry out other tasks; this will be dependent upon factors

such as workload and staffing levels:

a. To assess, diagnose and manage patients with minor illnesses and injuries

b. Manage patients presenting with a range of acute and chronic medical conditions,

providing subject matter expert advice to patients

c. Practice as an independent non-medical prescriber prescribing within current

legislation in line with medicines reconciliation

d. Identify, manage and support patients as risk of developing long-term conditions,

preventing adverse effects on the patients health

e. Recognise and work within own competence and professional code of conduct as

regulated by the NMC Ensure clinical practice is safe and effective and remains

within boundaries of competence. Acknowledge limitations of knowledge and skills,

refer and discuss with duty doctor as required.

f.Order and interpate results of tests/investigations and work collaboratively with

other healthcare professionals when needed

g. Making necessary onward referrals into secondary care and other

community providers

h. Support the Nursing team when required, offering support if needed

i. Attend and participate in relevant practice meetings, clinical supervision

with a Lead GP, and educational events

j. Maintain accurate clinical records in conjunction with extant legislation

k. Ensure read codes and templates are used effectively

l. Maintain chronic disease registers

m. Chaperone patients where necessary

n. Prioritise health issues and intervene appropriately

o. Support the team in dealing with clinical emergencies

p. Contribute to practice targets (QOF etc.), complying with local and regional guidance

q. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

r. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

s. Support the clinical team with all safeguarding matters, in accordance with local and national policies

t. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

u. Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the Advanced Nurse Practitioner may be

requested to:

a. Support and participate in shared learning within the practice

b. Continually review clinical practices, responding to national policies and initiatives where appropriate

c. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA)

d. Develops practice administrative and clinical protocols in line with the needs of the patient and current legislation
