Job summary

Lathom House Surgery is a semi-rural practice located in the village of Burscough, West Lancashire, approximately 16 miles north east of Liverpool and 9 miles east of Southport. We have been established for 27 years.

We are offering a rare and unique opportunity to join our organisation in a pivotal role within our team.

We are looking for a highly motivated, enthusiastic and dynamic Practice Nurse to join our team providing patient centred care in a modern, forward thinking practice.

We have excellent core values in our approach to patient centred care, well established chronic disease management programme and a very experienced ANP/HCA team with support from senior GPs, Nurses and Clinical Pharmacist.

Main duties of the job

    • Qualifications and experience in Cytology, Immunisations, and Chronic Disease Management including Asthma, Hypertension, COPD, and Diabetes and preferably with Nurse Prescriber Qualification however training can be undertaken by the suitable candidate, support will be provided in house and future career development is also supported for our nursing staff.

    • Responsibility in coordinating care for patients. For example QOF, and being able to organise and direct patient reviews. Experience in drug shared care management and blood testing.

    • Practical skills such as Spirometry, Peak Flow Assessments, Blood Glucose, ECG, and Vaccine administration.

    • Ability to interpret blood test results, and hospital letters

    • Experience in quality improvement projects and audits

    • Highly developed interpersonal and communication skills

    • An enthusiastic and flexible approach to team working
