Job summary

This position is PART TIME 25hrs per week.

Boston PCN has a Wellbeing Team to support the holistic needs of identified cohorts of patients. The team is multidisciplinary and will work together to support the varied needs of this patient group as well as linking in with other local services and the third sector. In addition, we are growing our Home Visiting Service, which provides advanced nurse practitioner visits to people who have a same day need and cant get to their GP practice. We also work closely with our Neighbourhood Team.

These community-based teams work to provide care out in the community. They require an enthusiastic and organised coordinator to provide administrative support.

Job responsibilities

The team coordinator will provide administrative and coordination support to the multidisciplinary teams and patients.

Key outcomes of the teams are:

  • Holistic support for patients to better manage their condition
  • Supporting patients who will benefit from proactive health management
  • Enabling people to remain in, or close to, their own home, whenever possible
  • Improving pathways between services, reducing inefficiencies, fragmentation, and duplication of care
  • Promote and encourage access to community based public health initiatives such as healthy eating, physical activity and social connectedness
  • Individuals being better equipped to self care, relying less on GP and urgent care services
  • Supporting complex patients through a multidisciplinary based approach
  • Providing urgent on the day visits on behalf of GP practices
