Job summary

Boston Primary Care Network is recruiting Pharmacist(s) who are passionate about using their extensive skills and expertise to maximise the benefits patients receive from medications. You will join and further develop their forward-thinking Clinical Pharmacy and Medicines Optimisation team. The team is well established and valued within the PCN and includes a mix of Clinical Pharmacists, a Pharmacy Technician, and an administrator, supported by a Lead Clinical Pharmacist and wider PCN leadership team.

We will support you to develop your role and skills, including but not limited to the following;

· Enrolment on the General Practice Pharmacist Training Pathway from CPPE (unless an equivalent qualification is already held and exemption is agreed by CPPE)

· Becoming an independent prescriber (if the qualification is not already held)

· You will have clinical supervision from a GP within the PCN

· Overall direction and support will be provided by the established Lead Clinical Pharmacist

· Peer support is vital for this role and will be provided by Clinical Pharmacy colleagues within the PCN and other Clinical Pharmacists working in similar roles for other networks

Main duties of the job

The ideal candidate(s) will relish the opportunity to work independently as required but also collaboratively with various members of the MDT across the PCN including GP’s, nurses, pharmacy colleagues, physiotherapists, occupational therapists and paramedics. They will use an individualised approach to proactively achieve medicines optimisation for patients within the Boston Primary Care Network.

Applications are welcome from pharmacists with patient-facing experience from any sector. Whilst relevant previous experience in general practice is desirable, this is not essential; the structured training pathway provided by CPPE coupled to the support provided within the PCN by Clinical Pharmacist colleagues will enable candidates committed to continual development to be successful in the role.

For candidates that are already independent prescribers, you will be supported further with development and education via the Advanced Clinical Practice pathway established by Health Education England as appropriate.

This is an exciting opportunity for Clinical Pharmacists to develop their own role as they complete the CPPE Primary Care Pharmacy Education Pathway. As you develop your clinical assessment skills there will be an expectation to conduct face to face patient reviews to optimise patient care accordingly.

Essential

  • In depth therapeutic and clinical knowledge of the principles of evidence-based healthcare
  • An appreciation of the nature of primary care prescribing, concepts for rational prescribing and strategies for improving prescribing
  • Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports

Desirable

  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
  • Able to manage, plan, monitor and review general medicines optimisation issues in core areas for long term conditions

Qualifications

Essential

  • Completion of an undergraduate degree in Pharmacy
  • Demonstrates an understanding of, and conforms to, relevant standards of practice

Desirable

  • Postgraduate Diploma in Clinical Pharmacy
  • Independent Prescribing Qualification

Experience

Essential

  • Minimum of two years experience as a Pharmacist
  • Experience and awareness of the breadth of common acute and long term conditions that are likely to be encountered in Primary care

Desirable

  • Demonstrates ability to integrate Primary Care with community and hospital pharmacy teams and other care providers across the PCN
  • Experience of partnership/collaborative working and building relationships across organisations
