Boston Primary
Care Network (PCN) is a forward thinking and dynamic group of 6 GP practices.
We have a fantastic opportunity for a Health Inclusion Officer to join the PCN
to support the Inclusion Health groups in the community.
The four groups
originally defined under the term Inclusion Health includes Gypsies, Roma and
Travellers, homeless people, vulnerable migrants and sex workers. However, the
term is widely used to refer to populations at the sharp edge of health
inequalities as a result of social exclusion and stigmatisation. This includes
people in contact with the criminal justice system, people with learning
disabilities and mental health needs and more.
Inclusion Health groups persistently face some of the
greatest difficulties in accessing primary healthcare services and continue to
experience the poorest health outcomes and life expectancies across the
population. You will have the opportunity to bring your ideas to develop a
clear agenda and strategy to address the issues and barriers that inclusion
health groups face. As a Health
Inclusion Officer you will drive forward changes that will improve access to
services for inclusion health groups and help to meet targets for the Network
contract DES.
Main duties of the job
To develop the PCNs understanding of inclusion health
groups. Seeking further insight into the inclusion health groups in the local
population and their needs.
Understand key barriers for all health inclusion
groups and how the PCN can help to address these.
Highlight key priorities for the residents of inclusion
health groups to the PCN board.
Work with critical stakeholders such as Boston Borough
Council, charities and other community groups.
Develop and use Patient Participation groups
effectively and recruit from diverse communities.
Ensure the PCN has access to up-to-date and
comprehensive advice and templates to support people with benefit claims,
housing claims and other things that support people with issues they are
experiencing across the wider determinants of health.
Ensure all staff are aware of patient entitlements and
provide staff training when necessary to develop their knowledge of inclusion
health groups.
Understand diverse cultural backgrounds of the local population
Work
with communications teams to advocate suitable messages and engagement
with seldom heard and potentially excluded groups.
Interact with people
at all levels and from a range of backgrounds.
