Job summary

Skelmanthorpe Family Doctors are looking for an Advanced Clinical Practitioner to replace a member of staff that is leaving. We are patient centred practice with the patient at the heart of what we do. We are looking somebody who wants to be part of team that works well, the job can be varied with Care Home MDTs, On-call, Palliative Care etc etc.

Main duties of the job

Main duties will be seeing patients either F2F, telephone or Home Visits. Supporting the practice with QOF, DEs's and LES's and any new areas of work that may come along. Being part of a ACP/ANP team that support the partners in looking after the patients.

About us

We are 9500 patient practice with three partners, 2 salaried GP's, ACP/Clinical Pharmacist,ANP and Trainee ACP. 3 Practice nurses, 2 Nursing Associates, 3 HCA's and a Phlebotomist. This is a settled team with a mixture of youth and experience. Some staff have been at the organisation for many years, all staff are very supportive in their roles to other members of staff.

Job description

Job responsibilities

An experienced ANP-ACP who can work independently and collaboratively within the clinical team.

A professional with highly developed clinical assessment and diagnostic skills.

Someone who applies critical thinking to deliver safe and effective care within their professional scope.

A proactive clinician who can help manage patient demand and support patients in making informed choices about their care.

Providing clinical care to a diverse patient population.

Using advanced clinical skills to assess, diagnose, treat, and manage patients.

Supporting the wider practice team in delivering responsive and efficient services

Work within clinical governance frameworks to ensure the highest standards of patient care, adhering to CQC guidance and evidence-based best practice.

Complete and maintain all mandatory training requirements in a timely and consistent manner.

Take responsibility for assessing, diagnosing, planning, implementing, and evaluating treatment/interventions for patients with undifferentiated and undiagnosed conditions, including initiating appropriate referrals and further investigations.

Assess and manage patients' physical, psychological, and social needs, ensuring appropriate treatment, discharge, or referral pathways.

Deliver assessment, diagnosis, and treatment at the point of first contact, ensuring safe and effective care.

Clinical Practice

Prescribe medication independently as a qualified Non-Medical Prescriber, following a comprehensive clinical examination and in accordance with current clinical guidelines.

Maintain comprehensive, accurate, and timely patient records in line with relevant legislation, policies, and procedures.

Promote and ensure effective communication within the practice and with external healthcare partners and stakeholders.

Participate in and contribute to practice meetings, clinical audits, and service development activities.

Identify and support opportunities for collaborative working across organisations to improve continuity and coordination of care.

Support and actively engage in shared learning, mentoring, and best practice dissemination across the practice and wider healthcare network.

Perform home visits where required.

Communicate effectively across all levels of the Practice and other healthcare organisations to ensure a consistent, patient-centred service.

Work closely with multi-disciplinary team members, including doctors, nurses, and allied health professionals, to ensure integrated care delivery.

Accept referrals from colleagues and refer patients appropriately to specialists and services in line with clinical pathways and service provision..
